Function and Efficacy

In this product, ampicillin sodium is a broad-spectrum semi-synthetic penicillin, and cloxacillin sodium is an acid-resistant and enzyme-resistant semi-synthetic penicillin. This product has the characteristics of both ampicillin sodium and cloxacillin sodium. It has a broad-spectrum killing effect on Gram-positive and Gram-negative bacteria, and is effective against penicillin-resistant Staphylococcus aureus. The combination of the two can enhance each other's effects and make up for the shortcomings of their individual use. Ampicillin has a strong antibacterial effect on hemolytic streptococci, pneumococci, and non-penicillinase-producing staphylococci, which is similar to or slightly inferior to penicillin. Ampicillin also has a good antibacterial effect on viridans streptococci, and its effect on enterococci and Listeria is better than penicillin. Ampicillin sodium has antibacterial activity against Corynebacterium diphtheriae, Bacillus anthracis, Actinomyces, Haemophilus influenzae, Bordetella pertussis, Neisseria, and anaerobic bacteria except Bacteroides fragilis. Some Proteus mirabilis, Escherichia coli, Salmonella, and Shigella are sensitive to this product. It is ineffective against methicillin-resistant Staphylococcus aureus. Ampicillin exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. Cloxacillin is acid-resistant and penicillinase-resistant, and has antibacterial activity against Gram-positive cocci and Neisseria. Its antibacterial activity against enzyme-producing strains of Staphylococcus aureus (including Staphylococcus aureus and coagulase-negative Staphylococcus aureus) is stronger than that of oxacillin, but its antibacterial effect on penicillin-sensitive Staphylococci and various streptococci is weaker than that of penicillin, and it is ineffective against methicillin-resistant Staphylococcus aureus.

Appearance

This product is white or off-white powder or crystalline powder.

Indication

It is suitable for respiratory tract infections, gastrointestinal infections, urinary tract infections, soft tissue infections, endocarditis, meningitis, sepsis, etc. caused by sensitive bacteria. It can also be used for mixed infections caused by Streptococcus pyogenes or Pneumococcus and penicillin-resistant Staphylococcus.

Usage and Dosage

1. Intramuscular injection. 0.5-1.0g at a time, 3-4 times a day, add 2-4ml of sterile injection water to each bottle to dissolve, and inject deep into the muscle. 2. Intravenous drip. 2-4g per day, divided into 2-4 doses, can be compatible with various infusions, but if it is compatible with glucose infusion, it should be dripped at a faster speed, and the drip should be completed within half an hour to avoid reducing the efficacy. Children should be given 20-40mg/kg per day according to their body weight, divided into doses.

Adverse Reactions

1. The adverse reactions of this product are similar to those of penicillin, with allergic reactions being more common. The most common reaction is rash, which often occurs 5 days after medication, in the form of urticaria or maculopapular rash; interstitial nephritis may also occur; anaphylactic shock is occasionally seen, and once it occurs, it must be rescued on the spot. 2. Granulocytopenia and thrombocytopenia are occasionally seen in patients using ampicillin sodium. 3. Antibiotic-related enteritis is rare. 4. A small number of patients experience elevated serum transaminases and cholestatic jaundice. 5. Infants may experience increased intracranial pressure after using ampicillin sodium, manifested as a bulge in the anterior fontanelle.

Precautions

1. Patients with a history of allergy to penicillins or a positive penicillin skin test are contraindicated. 2. Breastfeeding women are contraindicated.

Special Population Medication

Precautions during pregnancy and lactation: It is forbidden to use during lactation.

Drug Interactions

1. This product is incompatible with aminoglycosides, norepinephrine, metaraminol, phenobarbital, vitamin B group, vitamin C and other drugs, and should not be dripped in the same bottle. 2. Probenecid can reduce the renal tubular secretion of cloxacillin and prolong the serum half-life of this product. 3. Aspirin and sulfonamides inhibit the binding of this product to serum proteins and increase the free blood concentration of this product. 4. Ampicillin and kanamycin have synergistic antibacterial effects on Escherichia coli and Proteus. 5. This product should be dripped alone and should not be dripped in the same bottle with the following drugs: aminoglycosides, clindamycin phosphate, lincomycin hydrochloride, polymyxin B, succinic chloramphenicol, erythromycin, epinephrine, metaraminol, dopamine, atropine, calcium gluconate, vitamin B group, vitamin C, nutrient injections containing amino acids and hydrocortisone succinate, etc. 6. Allopurinol can increase the incidence of ampicillin rash reactions, especially in patients with hyperuricemia. 7. Ampicillin can stimulate estrogen metabolism or reduce its enterohepatic circulation, thus reducing the effectiveness of oral contraceptives.
