Trazodone Hydrochloride Tablets

Function and Efficacy

Trazodone hydrochloride belongs to the triazolopyridine class of antidepressants. Its exact antidepressant mechanism for humans has not yet been fully elucidated, but it is generally believed that at therapeutic doses, its antidepressant pharmacological effect lies in the selective inhibition of the reabsorption of 5-hydroxytryptamine (5-HT), and can weakly prevent the reabsorption of norepinephrine, but has no effect on dopamine, histamine and acetylcholine, nor does it inhibit the activity of monoamine oxidase (MAO) in the brain. Trazodone hydrochloride also has a reinforcing effect on behavioral changes induced by 5-HT's precursor 5-hydroxytryptophan. Its antidepressant effect is similar to that of tricyclics and monoamine oxidase inhibitors (MAOIs), but it has less toxicity to the cardiovascular system and is more suitable for the elderly or patients with cardiovascular diseases. This product also has central sedative effects and slight muscle relaxant effects.

Appearance

This product is white or off-white tablets.

Indication

Suitable for all types of depression.

Usage and Dosage

The dosage should start at a low dose, gradually increase the dose and observe the therapeutic response. When drowsiness occurs, most of the daily dose must be distributed to bedtime or reduced. Symptoms will be relieved within the first week of medication, and the best antidepressant effect will appear within two weeks. It usually takes two to four weeks to achieve the best effect. The recommended initial dose for adults is 50~100mg (1-2 tablets) per day, taken in divided doses, and then 50mg (1 tablet) can be increased every three to four days. For outpatients, 200mg (4 tablets) per day is generally appropriate, taken in divided doses. The dose can be larger for more serious inpatients. The maximum dose does not exceed 400mg (8 tablets) per day, taken in divided doses. The maintenance dose for long-term use should be kept at the lowest dose. Once there is sufficient efficacy, the dose can be gradually reduced.

Adverse Reactions

Common adverse reactions include drowsiness, fatigue, dizziness, headache, insomnia, tension and tremor, as well as blurred vision, dry mouth and constipation. Rarely, postural hypotension (which can be alleviated by taking the drug at the same time as meals), tachycardia, nausea, vomiting and abdominal discomfort occur. Very few patients experience musculoskeletal pain and dreaminess. In clinical studies, some adverse reactions have been reported that may be related to the use of trazodone hydrochloride: akathisia, allergic reactions, anemia, flatulence, abnormal urination, sexual dysfunction and menstrual abnormalities. However, these are seen in very few patients.

Precautions

It is contraindicated for those who are allergic to trazodone hydrochloride, those with severe liver damage, severe heart disease or arrhythmia, and those with impaired consciousness.

Special Population Medication

Precautions for children: Not clear yet. Precautions for pregnancy and lactation: For pregnant women, there is not enough and well-controlled research evidence for this product, so use it with caution. There is a small amount of trazodone hydrochloride and its metabolites in breast milk, so use it with caution in lactating women. Precautions for the elderly: Trazodone hydrochloride has fewer side effects on heart disease and has very weak peripheral anticholinergic effects, so it is more suitable for elderly patients.

Drug Interactions

1. It should not be used in combination with MAO inhibitors. 2. When used in combination with digoxin or phenytoin, the serum concentration of digoxin or phenytoin may be increased. When combined use is required, the blood concentrations of the two drugs should be closely monitored.
