Norepinephrine bitartrate injection

Function and Efficacy

This product is an adrenaline receptor agonist. It is a strong α receptor agonist and also stimulates β receptors. Through α receptor stimulation, it can cause extreme vasoconstriction, increase blood pressure, and increase coronary blood flow; through β receptor stimulation, it can strengthen myocardial contraction and increase cardiac output. When the dosage is 0.4μg/kg per minute, β receptor stimulation is the main effect; when a larger dose is used, α receptor stimulation is the main effect.

Indication

Used to treat hypotension caused by acute myocardial infarction, extracorporeal circulation, etc.; for shock, hypotension or hypotension after pheochromocytoma resection caused by insufficient blood volume, this product is used as an auxiliary treatment to supplement blood volume during emergency treatment to restore blood pressure and temporarily maintain brain and coronary artery perfusion until the blood volume supplementation treatment takes effect; it can also be used for hypotension during intrathecal blockade and blood pressure maintenance after cardiac arrest resuscitation.

Usage and Dosage

Dilute with 5% glucose injection or glucose sodium chloride injection and drip intravenously. 1. Common dosage for adults: Start with a drip rate of 8-12 μg per minute, and adjust the drip rate to achieve the ideal blood pressure level; the maintenance dose is 2-4 μg per minute. If necessary, the above dose can be exceeded according to the doctor's instructions, but attention should be paid to maintaining or replenishing blood volume. 2. Common dosage for children: Start with a drip rate of 0.02-0.1 μg/kg per minute according to body weight, and adjust the drip rate as needed.

Adverse Reactions

1. Leakage of the drug solution may cause local tissue necrosis. 2. The strong vasoconstriction of this product can reduce the blood flow of important organs. After the renal blood flow decreases sharply, the urine volume decreases, and the insufficient blood supply to the tissues leads to hypoxia and acidosis. When used for a long time or in large quantities, it can reduce the return blood flow to the heart, increase the peripheral vascular resistance, and reduce the cardiac output, with serious consequences. 3. Reactions that should be paid attention to include whitening of the skin along the venous route during intravenous infusion, local skin rupture at the injection site, cyanosis of the skin, redness, and severe dizziness. Although the above reactions are rare, the consequences are serious. 4. Some patients have rashes and facial edema due to allergies. 5. Arrhythmias may occur in patients with hypoxia, electrolyte imbalance, organic heart disease, or when the dosage is exceeded; reflex heart rate slowing may occur after blood pressure rises. 6. If the following reactions continue to occur, attention should be paid: anxiety, dizziness, headache, pale skin, palpitations, insomnia, etc. 7. Severe headaches and hypertension, slow heart rate, vomiting, and convulsions may occur when the dosage is exceeded.

Precautions

It is forbidden to use it in combination with halogenated anesthetics and other catecholamines. It is contraindicated in patients with cocaine poisoning and tachycardia.

Drug Interactions

When used with general anesthetics such as chloroform, cyclopropane, and halothane, the myocardium may be more sensitive to sympathomimetic amines, and ventricular arrhythmias may occur easily. They should not be used together. If they must be used together, the dosage should be reduced. 1. When used with beta-blockers, the efficacy of each is reduced. After beta-receptor blockade, the effect of alpha receptors is prominent, and hypertension and bradycardia may occur. 2. When used with antihypertensive drugs, it can offset or weaken the effect of antihypertensive drugs. When used with methyldopa, the pressor effect of this product is also enhanced. 3. When used with digitalis, it is easy to cause arrhythmias, and close attention should be paid to ECG monitoring. 4. When used with other sympathomimetic amines, the cardiovascular effect is enhanced. 5. When used with ergot preparations such as ergotamine, ergonovine, or oxytocin, it can enhance vasoconstriction and cause severe hypertension and bradycardia. 6. When used in combination with tricyclic antidepressants, the cardiovascular effects of this product may be enhanced by inhibiting tissue absorption of this product or enhancing the sensitivity of adrenaline receptors, causing arrhythmia, tachycardia, hypertension or hyperthermia. If it must be used in combination, the initial dosage of this product should be small and the cardiovascular effects should be monitored. 7. When used in combination with thyroid hormones, the effects of both are enhanced. 8. When used in combination with tolazoline, it may cause a drop in blood pressure, followed by an excessive rebound increase in blood pressure. Therefore, this product should not be used when tolazoline is overdosed.
