Function and Efficacy

1 Inhibitory effect on bladder micturition movement in vivo (1) Increase bladder capacity Cystometritis showed that this drug can increase the maximum bladder capacity of anesthetized rats and dogs, as well as the maximum bladder capacity and effective bladder capacity of decapitated dogs, but no significant increase in residual urine volume was observed. (2) Inhibit micturition movement This drug can reduce the number of rhythmic contractions (micturition movements) during bladder filling in anesthetized rats and dogs. (3) Inhibit bladder contraction caused by electrical stimulation Regardless of whether the pelvic nerve is cut, this drug can reduce the contraction force of the bladder caused by electrical stimulation of the dog's pelvic nerve. 2 Effect on isolated bladder It can inhibit the contraction of bladder smooth muscle caused by acetylcholine, calcium chloride (rats, dogs, guinea pigs), and electrical stimulation (rats, dogs, rabbits) in a dose-related manner. 3 Mechanism of action It can inhibit the contraction of acetylcholine and calcium chloride on isolated bladder, has affinity for myosin receptors, and can inhibit the contraction caused by electrical stimulation that atropine cannot inhibit. It can also inhibit bladder contraction caused by stimulation of cut pelvic nerve endings, indicating that this drug can directly act on bladder smooth muscle. In addition, the main metabolite of this drug, 1-methyl-4-piperidinyl-diphenylpropoxyacetic acid-N-oxide (N-oxide of this drug, hereinafter referred to as M-1) directly acts on smooth muscle, and 1-methyl-4-piperidinyl-benzoic acid-N-oxide (depropylated derivative of M-1, hereinafter referred to as M-2) has an anticholinergic effect. Therefore, this drug can directly act on smooth muscle and has an anticholinergic effect, but it is mainly because it directly inhibits smooth muscle and inhibits urination movement.

Appearance

Indication

This product is effective for methemoglobinemia caused by chemicals such as nitrite, nitrate, aniline, nitrobenzene, trinitrotoluene, benzoquinone, phenylhydrazine, etc. and drugs containing or producing aromatic amines (acetanilide, acetaminophen, phenacetin, benzocaine, etc.). It is less effective for methemoglobinemia caused by congenital reduced diphosphate pyridine nucleoside methemoglobin reductase deficiency. It is ineffective for abnormal hemoglobin M accompanied by methemoglobinemia. It can temporarily delay the toxicity of acute cyanide poisoning.

Usage and Dosage

Intravenous injection: 1-2 mg/kg of body weight for nitrite poisoning, 5-10 mg/kg of body weight for cyanide poisoning, and the maximum dose is 20 mg/kg.

Adverse Reactions

Intravenous injection of this product too quickly can cause dizziness, nausea, vomiting, chest tightness, abdominal pain, and excessive dosage. In addition to the aggravation of the above symptoms, headaches, lowered blood pressure, increased heart rate with arrhythmia, profuse sweating and impaired consciousness may also occur. After taking the drug, the urine will be blue and there may be stinging pain in the urethra during urination.

Precautions

This product is contraindicated in the following cases: 1. Patients with pyloric, duodenal and intestinal occlusion (inhibiting gastrointestinal smooth muscle contraction and peristalsis may aggravate symptoms). 2. Patients with lower urinary tract occlusion (relaxing the detrusor muscle may aggravate symptoms). 3. Patients with glaucoma (anticholinergic effect increases intraocular pressure and aggravates symptoms). 4. Patients with severe heart disease (literature reports that this drug can cause extrasystoles of the heart, aggravating symptoms). 5. People who are allergic to this product.

Precautions for children: The effectiveness and safety of this product for children have not been established, and clinical use is not recommended. Precautions for pregnancy and lactation: The effectiveness and safety of this product for pregnant women have not been established; animal experiments have shown that this product can be detected in breast milk, and lactating women should stop breastfeeding when using it. Precautions for the elderly: Elderly patients often have impaired liver and kidney function. For safety reasons, they should start with 10 mg/day.
