Function and Efficacy
1. Inhibition of bladder micturition movement in vivo (1) Increased bladder capacity Cystometritis showed that this drug could increase the maximum bladder capacity of anesthetized rats and dogs, as well as the maximum bladder capacity and effective bladder capacity of debulked dogs, but no significant increase in residual urine volume was observed. (2) Inhibition of micturition movement This drug could reduce the number of rhythmic contractions (micturition movement) during bladder filling in anesthetized rats and dogs. (3) Inhibition of bladder contraction induced by electrical stimulation This drug could reduce the contraction force of the bladder induced by electrical stimulation of the pelvic nerve in dogs, regardless of whether the pelvic nerve was cut off. 2. Effect on isolated bladder This drug could inhibit the contraction of bladder smooth muscle induced by acetylcholine, calcium chloride (rats, dogs, guinea pigs), and electrical stimulation (rats, dogs, rabbits) in a dose-related manner. 3. Mechanism of action This drug could inhibit the contraction of acetylcholine and calcium chloride on isolated bladder, have affinity for myosin receptors, and could inhibit the contraction induced by electrical stimulation that could not be inhibited by atropine. It could also inhibit the bladder contraction induced by stimulation of the cut pelvic nerve endings, indicating that this drug could directly act on bladder smooth muscle. In addition, the main metabolite of this drug, 1-methyl-4-piperidinyl-diphenylpropoxyacetic acid-N-oxide (N-oxide of this drug, hereinafter referred to as M-1) directly acts on smooth muscle, and 1-methyl-4-piperidinyl-benzoic acid-N-oxide (depropylated derivative of M-1, hereinafter referred to as M-2) has an anticholinergic effect. Therefore, this drug can directly act on smooth muscle and has an anticholinergic effect, but it is mainly because it directly inhibits smooth muscle and inhibits urination movement.

Appearance

Indication
Frequent urination and urinary incontinence caused by the following diseases or conditions: neurogenic bladder, neurogenic urinary incontinence, unstable bladder, irritative bladder conditions (chronic cystitis, chronic prostatitis).
Usage and Dosage
Oral administration: Adults generally take 20 mg (2 tablets) once a day after meals.

Precautions
(1) Patients with urinary retention, gastric retention, intestinal obstruction, and uncontrolled angle-closure glaucoma are contraindicated. (2) Patients with severe heart disease (literature reports that this drug can cause premature heart contractions, aggravating symptoms). (3) Patients who are allergic to this product.
Special Population Medication
Precautions for children: The effectiveness and safety of children's use have not been established, and clinical use is not recommended. Precautions for pregnancy and lactation: Not yet clear. Precautions for the elderly: Elderly patients often have impaired liver and kidney function. For safety reasons, 10 mg/day should be started.
Drug Interactions
This product should not be used in combination with the following drugs: anticholinergics, tricyclic depressants, phenothiazines, monoamine oxidase inhibitors (may enhance anticholinergic effects).
