Function and Efficacy

1. Complexation. ① Heavy metal poisoning: this product can complex heavy metals such as copper, iron, mercury, lead, and arsenic to form stable and soluble complexes that are excreted in urine. Its lead removal effect is not as good as that of calcium sodium edetate, and its mercury removal effect is not as good as that of dimercaprol; but this product can be taken orally, with slightly fewer adverse reactions, and can be used for mild heavy metal poisoning or when other complexing agents are contraindicated. ② Wilson's disease is a common chromosomal recessive genetic disease, mainly with a large amount of copper deposited in the liver and brain tissues, causing lenticular degeneration and cirrhosis. This product can combine with copper deposited in the tissues to form a soluble complex that is excreted in urine. ③ Cystinuria and its stones: this product can react with cystine to form a mixture of cysteine-penicillamine disulfide, thereby reducing the concentration of cystine in urine; the solubility of this mixture is 50 times greater than that of cystine, so it can prevent the formation of cystine stones; long-term use for 6 to 12 months may gradually dissolve the already formed cystine stones. 2. Anti-rheumatoid arthritis: The mechanism of action for the treatment of rheumatoid arthritis is not yet clear. After taking the drug, it was found that the lymphocyte function was improved, and the levels of IgM rheumatoid factor and immune complex in serum and joint capsule fluid were significantly reduced, but there was no significant reduction in the absolute value of serum immunoglobulin. It inhibited the activity of T cells in vitro, but had no effect on B cells. This product can also inhibit the cross-linking of newly synthesized procollagen, so it is also used to treat skin and soft tissue collagen diseases.

Appearance

This product is a coated tablet. After removing the coating, the tablet core is white.

Indication

1. Essential hypertension: It can be used alone or in combination with other antihypertensive drugs, especially thiazide diuretics. 2. Heart failure: Mild or moderate heart failure (NYHA grade II or III), combined with digitalis, diuretics and angiotensin converting enzyme inhibitors (ACEI). It can also be used for patients with heart failure who are intolerant to ACEI and who are treated with or without digitalis, hydralazine or nitrates.

Usage and Dosage

Adult conventional dose: Oral administration is generally 1 gram (8 tablets) per day, divided into 4 times. 1. Hepatolenticular degeneration, rheumatoid arthritis: 125-250 mg (1-2 tablets) per day at the beginning, and then increase by 125-250 mg (1-2 tablets) every 1-2 months. The commonly used maintenance dose is 250 mg (2 tablets) once, 4 times a day. The maximum daily dose is generally not more than 1.5 grams (12 tablets). When the symptoms improve and blood copper and ceruloplasmin reach normal, the dose can be halved to 500-750 mg (4-6 tablets) per day or intermittent medication. If the treatment is still ineffective after 3-4 months, other drugs should be used for treatment. 2. Heavy metal poisoning: 1-1.5 grams (8-12 tablets) per day, divided into 3-4 times, 5-7 days as a course of treatment; after stopping the drug for 3 days, the next course of treatment can be started. Generally, 1-4 courses of treatment are required depending on the amount of toxins in the body.

Adverse Reactions

1. Common symptoms include anorexia, nausea, vomiting, active ulcer disease, stomatitis and ulcers. 20% of the patients have abnormal taste. 2. Allergic reactions include skin itching, urticaria, fever, joint pain and lymphadenopathy. Other skin reactions include lupus erythema and pemphigus-like skin lesions. 3. This product inhibits the cross-linking of procollagen, making the skin brittle and bleeding, and affecting wound healing. 4. A small number of patients develop leukopenia, and other hematopoietic system damage includes agranulocytosis, aplastic anemia, eosinophilia, hemolytic anemia and thrombocytopenic purpura. 5. 6%-20% of the patients develop proteinuria, sometimes hematuria and nephrotic syndrome caused by immune complex membrane glomerulonephritis. 6. Some patients develop alopecia, bile retention, Goodpasture syndrome, myasthenia gravis and tinnitus, and laboratory tests show decreased IgA. Most adverse drug reactions automatically relieve and disappear after discontinuation of the drug. Allergic reactions are effectively treated with adrenocortical hormones and antihistamines. For abnormal taste, except for patients with Wilson's disease, 5 to 10 drops of 4% copper sulfate solution can be added to juice and taken orally twice a day to help restore taste.

Precautions

1. It is contraindicated for patients with renal insufficiency, pregnant women and those allergic to penicillins. 2. It is contraindicated for patients with agranulocytosis and aplastic anemia.

Special Population Medication

Precautions for children: This experiment has not been conducted and there is no reliable reference. Precautions for pregnancy and lactation: Pregnant women are prohibited to use. Precautions for the elderly: Elderly people over 65 years old are prone to hematopoietic system toxicity when taking it.

Drug Interactions

This product can aggravate the adverse reactions of antimalarial drugs, gold preparations, immunosuppressants, and phenylbutazone on the hematopoietic system and kidneys. For patients taking oral iron preparations, this product should be taken 2 hours before taking iron preparations to avoid weakening the efficacy of this product.
