Function and Efficacy
This product is a medium-acting anticoagulant of the dicoumarin class. Its mechanism of action is to competitively counteract the effect of vitamin K, inhibit the synthesis of coagulation factors in hepatocytes, and also has the effect of reducing thrombin-induced platelet aggregation reaction, thus having anticoagulant and antiplatelet aggregation functions.

Appearance
This product is a sugar-coated tablet, which appears white after removing the sugar coating.
Indication
Suitable for patients who need long-term continuous anticoagulation: ① It can prevent the formation and development of blood clots and is used to treat thromboembolic diseases; ② It can treat venous thrombosis after surgery or trauma, and can be used as an auxiliary medication for myocardial infarction; ③ It can be used as a preventive medication for patients who have had thromboembolic diseases and those who are at risk of postoperative thrombotic complications.
Usage and Dosage
Oral administration, common dosage for adults: Avoid shock treatment, take 3-4 mg orally on the first to third days (half the dose is sufficient for the elderly, the weak and diabetic patients), and after three days, give a maintenance dose of 2.5-5 mg per day (the dose can be adjusted with reference to the coagulation time to make the INR value reach 2-3). Because this product takes effect slowly, a transient hypercoagulable state may exist in the first three days of treatment due to the inhibition of plasma anticoagulant protein cells. If an anticoagulant effect is required immediately, heparin can be used at the same time at the beginning, and heparin can be discontinued after this product fully exerts its anticoagulant effect.
Adverse Reactions

Precautions
1. It is contraindicated for patients with liver and kidney damage, severe hypertension, coagulation disorder with bleeding tendency, active ulcer, trauma, threatened abortion, and recent surgery. 2. It is contraindicated during pregnancy. 3. It should be used with caution in the elderly or during menstruation.
Special Population Medication
Precautions for children: The dosage should be adjusted according to individual needs. Precautions for pregnancy and lactation: 1. It is easy to pass through the placenta and cause teratogenesis. The use of this product during pregnancy can cause fetal warfarin syndrome, with an incidence of 5% to 30%. It manifests as epiphyseal separation, nasal hypoplasia, optic atrophy, mental retardation, heart, liver, spleen, gastrointestinal tract, head and other malformations. Use in late pregnancy can cause bleeding and stillbirth, so this product is contraindicated in the first 3 months of pregnancy and the third month of pregnancy. Pregnant women with hereditary thrombophilia can be given a small dose of heparin and receive close laboratory monitoring when using this product for treatment. 2. A small amount of warfarin can be secreted by breast milk. Lactating women take 5 to 10 mg per day, and the blood drug concentration is generally 0.48 to 1.8 mu; g/ml. The drug concentration in breast milk and infant plasma is extremely low, and the effect on infants is small. Precautions for the elderly: The elderly should use it with caution, and the dosage should be appropriately reduced and individualized.
Drug Interactions
1. Drugs that enhance the anticoagulant effect of this product include: aspirin, sodium salicylate, glucagon, quinidine, indomethacin, phenylbutazone, quinine, uric acid, tolbutamide, metronidazole, allopurinol, erythromycin, chloramphenicol, certain aminoglycoside antibiotics, cephalosporins, beniodarone, cimetidine, clofibrate, dextrothyroxine, acetaminophen, etc. 2. Drugs that reduce the anticoagulant effect of this product include: phenytoin sodium, barbiturates, oral contraceptives, estrogen, cholestyramine, rifampicin, vitamin K, chlorthalidone, spironolactone, primidone, corticosteroids, etc. 3. Drugs that cannot be used in combination with this product include: epinephrine hydrochloride, amikacin, vitamin B12, metaraminolamine, oxytocin, chlorpromazine hydrochloride, vancomycin hydrochloride, etc. 4. When this product is used in combination with chloral hydrate, its efficacy and toxicity are enhanced, so it should be used with caution in a reduced dose. 5. Impaired absorption or decreased synthesis of vitamin K also affects the anticoagulant effect of this product.
