Appearance
This product is an off-white cream with an emulsion base.
Usage and Dosage
Topical medication: The usual dose is 1 to 2 times a day. Clean and dry the affected area before use, then apply the cream thinly to the affected area and its surroundings and rub gently. If the affected area has been rubbed (such as under the breast, between the fingers, between the buttocks, and in the groin), gauze can be used to cover it after application, especially at night. Treatment course: 1 to 2 weeks for tinea corporis and tinea cruris, 2 to 4 weeks for tinea pedis, and 2 weeks for tinea versicolor.
Adverse Reactions
Occasionally, redness, itching, or stinging may occur at the site of application, but these symptoms rarely require discontinuation of the drug. Note: These symptoms must be distinguished from allergic reactions, which occur very rarely but require discontinuation of the drug.
Precautions
Contraindicated in patients allergic to terbinafine or any of the agents in the emulsion.
Special Population Medication
Precautions for children: There is limited experience with topical use of this product in children, so it is not recommended for use. Precautions for pregnancy and lactation: Animal studies have shown that this product has no adverse effects on the fetus, but it has not been verified by controlled trials in pregnant women, so it is not recommended for use by pregnant women unless the benefits far outweigh the risks. Terbinafine can be excreted in breast milk, although the amount is very small, but women who use this product should stop breastfeeding their babies. Precautions for the elderly: The dosage and adverse reactions of elderly patients are the same as those of young people.
Drug Interactions
Based on in vitro studies and studies in healthy volunteers, terbinafine slightly inhibits or increases the clearance of most drugs metabolized by the cytochrome P450 system (e.g., cyclosporine, terfenadine, triazoles, toluenesulfonyl urea, or oral contraceptives). However, in vitro studies have shown that terbinafine inhibits CYP2D6-mediated metabolism, and this in vitro finding may be clinically relevant for compounds that are primarily metabolized by this enzyme, such as tricyclic antidepressants (TCAs), beta-blockers, selective serotonin reuptake inhibitors (SSRIs), and monoamine oxidase inhibitors (MAO-Is) type B, as they also have a narrow therapeutic window. Cases of menstrual irregularities have been reported in patients taking terbinafine concomitantly with oral contraceptives.
