Function and Efficacy
The main pharmacological effects of anthralin include anti-epithelial cell proliferation, induction of epithelial cell differentiation and anti-inflammatory effects. Specifically, it blocks the pathological signal substances expressed by abnormal immune system in psoriasis by inhibiting the activity of protein kinase C in keratinocytes and polymorphonuclear leukocytes, inhibiting the secretion of interleukin-1, 6, 8 and tumor necrosis factor alpha by human peripheral blood monocytes; and by reducing the amount of transforming growth factor-alpha in keratinocytes and its affinity with epidermal growth factor receptors, thereby producing anti-keratinocyte proliferation and anti-inflammatory effects. It can cause proliferation of mouse tail granular cells and downregulate the expression of transglutaminase (TGase) mRNA, indicating that it has the effect of inducing epithelial cell differentiation. It produces anti-inflammatory effects by inhibiting the production of reactive oxygen species by polymorphonuclear leukocytes induced by phorbol esters; but it can also stimulate normal polymorphonuclear leukocytes to produce reactive oxygen species and cause inflammatory stimulation of normal skin. Inhibit the lipoxygenase metabolic pathway, reduce the production of 5-lipoxygenase products 5-HETE and leukotriene B4 in stimulated human polymorphonuclear leukocytes; inhibit leukocyte chemotaxis; inactivate mitochondria and inhibit cell respiration; inhibit glucose-6-phosphatase activity. All of the above inhibitory effects can produce anti-inflammatory effects.

Appearance
This product is a yellow ointment.
Indication
Mainly used for plaque psoriasis vulgaris.
Usage and Dosage
1. Concentration-escalating therapy: When starting treatment, use a low concentration for at least 5 days. After the skin has adapted, increase the concentration from 0.05%, 0.1%, 0.25%, 0.5%, 0.8%, 1.0% to 3%. Outpatients can be treated once a day, apply the medicine before going to bed, wash it off with soap the next morning, and apply moisturizer during the day to keep the skin lubricated. Inpatients can be treated twice a day, morning and evening. A tar bath before each treatment can increase the efficacy. 2. Short-term contact therapy: After testing different concentrations and contact times, it was found that a 3% concentration as the final dose, washing it off after 20 minutes of action, and treating once a day are the optimal concentrations and contact times; low-concentration, short-term contact therapy can also be used, that is, using 0.1% ointment for 5 to 20 minutes or using 1% ointment for 5 minutes and then
Adverse Reactions
The main adverse reactions are irritation to the skin, causing redness, burning, itching, etc. The nails can be dyed reddish brown and clothes can be stained yellow.
Precautions
It is contraindicated for patients allergic to anthracene compounds and for patients with progressive pustular psoriasis.
Special Population Medication
Precautions for children: Children must use under adult supervision. Precautions for pregnancy and lactation: It is not known whether anthralin will cause harm to the fetus or may affect fertility when used in pregnant women, so it is not recommended for use in pregnant women. There is no data to support the effects of anthralin on fetal growth, development and functional improvement. Precautions for the elderly: It is not clear yet.
Drug Interactions
Combined use with corticosteroids can reduce the irritation of anthralin and shorten the clearance period of skin lesions. However, due to the high recurrence rate of corticosteroids and the rebound of pustular psoriasis, the combined use of anthralin and corticosteroids is still worth considering. Combined use with urea can increase drug transdermal penetration and reduce the concentration of anthralin, thereby reducing inflammatory stimulation to the skin. Salicylic acid can prevent anthralin from being oxidized to anthrone and has the effect of protecting anthralin. Alkaline amines can inactivate anthralin by promoting its oxidation. After short-term contact treatment, applying fat-soluble amines can inhibit the inflammatory response caused by anthralin remaining in the stratum corneum.
Storage
This product is very easy to oxidize and change color. The raw materials and preparations should be stored in a cool and airtight place away from light.
