Meropenem for injection

Meropenem is a carbapenem antibiotic for parenteral use that is stable to human dehydropeptidase-1. It exerts its antibacterial and bactericidal effects by interfering with the synthesis of bacterial cell walls. This product easily penetrates the bacterial cell wall, is stable to most β-lactamases, and has a high affinity for penicillin binding proteins (PBPs). Therefore, meropenem has a broad-spectrum antibacterial activity against aerobic and anaerobic bacteria. The antibacterial range includes: Gram-positive aerobic bacteria; Staphylococcus aureus (including β-lactamase-producing strains); Staphylococcus epidermidis (including β-lactamase-producing strains); Streptococcus pneumoniae; Streptococcus pyogenes; Streptococcus agalactiae; Streptococcus viridans; Enterococcus faecalis; Enterococcus ureus; Nocardia; Listeria monocytogenes; Gram-negative

Meropenem

This product is white or slightly yellow crystalline powder; odorless, bitter taste.

Meropenem is indicated for infections in adults and children caused by single or multiple bacteria that are sensitive to meropenem. Pneumonia includes hospital-acquired pneumonia, urinary tract infection, gynecological infection (such as endometritis and pelvic inflammatory disease), skin and soft tissue infection, meningitis, and sepsis. For empirical treatment, this product can be used alone or in combination with antiviral or antifungal drugs for adult patients with neutropenia and fever. Meropenem alone or in combination with other antimicrobial agents can be used to treat multiple bacterial infections. There is currently no experience with the use of this product for infants with neutropenia or primary or secondary immunodeficiency.

Dosage: Adults: Dosage and time interval should be based on the type and severity of infection and the specific circumstances of the patient. The recommended daily dose is as follows: For pneumonia, urinary tract infection, gynecological infection (such as endometritis), skin or soft tissue infection, administer once every 8 hours, 500 mg each time, by intravenous drip. For the treatment of hospital-acquired pneumonia, peritonitis, combined infection, and sepsis in patients with neutropenia, administer once every 8 hours, 1 g each time, by intravenous drip. For patients with meningitis, it is recommended to administer once every 8 hours, 2 g each time. Children: For children aged 3 months to 12 years, depending on the severity of the infection type, the sensitivity of the pathogen, and the specific circumstances of the patient, a dose of 10 to 20 g is prescribed every 8 hours.

Serious adverse reactions are rare, and the following adverse reactions can be seen in clinical trials; * Inflammation at the injection site, thrombophlebitis, pain at the injection site; * Rash, itching, urticaria; * Abdominal pain, nausea, vomiting, diarrhea; * Reversible eosinophilia, thrombocytopenia, neutropenia. Direct or indirect Cooombs test is positive in some patients. * Reversible increase in serum bilirubin, transaminase, alkaline phosphatase, lactate dehydrogenase concentrations. * Headache, paresthesia; * Oral and vaginal candidal infections.

Patients who are allergic to this product are contraindicated.

When meropenem is used in combination with potentially nephrotoxic drugs, it should be noted that the combination of probenecid and meropenem can competitively activate renal tubular secretion, inhibit renal excretion, and lead to a prolonged elimination half-life of meropenem and an increase in blood drug concentration. Therefore, the combination of meropenem and probenecid is not recommended. When this product is used simultaneously with valproic acid, the blood drug concentration of valproic acid will be reduced, leading to epileptic seizures. Meropenem cannot be used simultaneously with valerate glyceride. Meropenem should not be used in combination with other drugs.
