Function and Efficacy

This product inhibits the transmembrane calcium ion influx of myocardial and vascular smooth muscle without changing the blood calcium concentration. Its effect on vascular smooth muscle is better than that on myocardium, so its vascular selectivity is relatively strong. In animal experiments, this product selectively dilates coronary vascular smooth muscle. The blood concentration when this effect occurs does not produce negative inotropic effect (myocardium), and has little effect on heart rhythm and cardiac contractility. In the human body, this product reduces peripheral vascular resistance. This effect is greater in patients with hypertension than in those with normal blood pressure, and there will be a reflex increase in heart rate when blood pressure is lowered. This product increases cardiac ejection fraction and cardiac output, while the left ventricular end-diastolic pressure does not change much. It can reduce myocardial oxygen consumption and total peripheral resistance, and can also increase coronary collateral circulation, thereby increasing coronary blood flow.

Indication

Treat hypertensive emergencies and emergency treatment of abnormally high blood pressure during surgery.

Usage and Dosage

1. First aid for abnormally high blood pressure during surgery: Intravenous drip, first give at a drip rate of 2-10μg/kg (body weight) per minute, and after the blood pressure drops to the target value, adjust the drip rate while monitoring the blood pressure. If it is necessary to quickly lower the blood pressure, nicardipine hydrochloride is administered intravenously at 10-30μg/kg (body weight). 2. Hypertensive emergency: Intravenous drip, first give at a drip rate of 0.5-6μg/kg (body weight) per minute, and after the blood pressure drops to the target value, adjust the drip rate while monitoring the blood pressure.

Adverse Reactions

1. Common symptoms include swollen feet, dizziness, headache, and flushed face, which are all the result of vasodilation. 2. Less common symptoms include palpitations, tachycardia, and aggravated angina pectoris, which are often the result of reflex tachycardia and can be corrected by reducing the dose or adding beta-blockers. 3. Less common symptoms include nausea, dry mouth, constipation, fatigue, rash, etc.

Precautions

1. Those who have allergic reaction to this product. 2. Severe aortic valve stenosis.

Drug Interactions

1 This product is well tolerated when used in combination with beta-blockers. 2 This product increases blood concentrations when used in combination with cimetidine. 3 This product does not increase blood concentrations of digoxin when used in combination with digoxin, but digoxin blood concentrations must be measured. 4 When this product is used in combination with cyclosporine, cyclosporine blood concentrations increase. 5 In vitro, therapeutic concentrations of furosemide, propranolol, dipyridamole, warfarin, quinidine, and naproxen added to human plasma do not change the protein binding rate of this product.
