Appearance

This product is a colorless or almost colorless clear liquid with a sweet taste.

Indication

1. Replenish energy and body fluids, used for insufficient food intake or large body fluid loss (such as vomiting, diarrhea, etc.) caused by various reasons, total intravenous nutrition, starvation ketosis. 2. Hypoglycemia; 3. Hyperkalemia; 4. Hypertonic solution used as tissue dehydrating agent; 5. Preparation of peritoneal dialysis solution; 6. Drug diluent; 7. Intravenous glucose tolerance test; 8. For the preparation of GIK (polarization solution) solution.

Usage and Dosage

(1) Energy supplementation When patients eat less or cannot eat for some reason, they can generally be given 25% glucose injection intravenously and replenish body fluids at the same time. The amount of glucose is calculated based on the required energy. (2) Total intravenous nutrition therapy Glucose is the most important energy supply substance in this therapy. In non-protein energy, the ratio of glucose to fat is 2:1. The specific amount depends on the clinical energy needs. According to the need for fluid replacement, glucose can be prepared into different concentrations of 25% to 50%. If necessary, insulin can be added. For every 5 to 10 g of glucose, 1 unit of regular insulin is added. Since the normal use of hypertonic glucose solution is highly irritating to the veins and requires the infusion of fat emulsion, large vein drip is generally used. (3) Hypoglycemia: For severe cases, 20 to 40 ml of 50% glucose injection can be given intravenously first. (4) Starvation ketosis: For severe cases, 5% to 25% glucose injection can be given intravenously. 100 g of glucose per day can basically control the condition. (5) Water loss: For isotonic water loss, 5% glucose injection is given intravenously. (6) For hyperkalemia, 10% to 25% injection solution is used. For every 2 to 4 g of glucose, 1 unit of regular insulin is added for infusion to reduce serum potassium concentration. However, this therapy only allows extracellular potassium ions to enter cells, and the total potassium content in the body remains unchanged. If potassium excretion measures are not taken, hyperkalemia may recur. (7) Tissue dehydration hypertonic solution (generally 50% glucose injection solution) is rapidly injected intravenously at 20 to 50 ml. However, the effect is short-lived. Clinically, attention should be paid to preventing hyperglycemia, and it is rarely used at present. When used to adjust the osmotic pressure of peritoneal dialysis fluid, 20 ml of 50% glucose injection solution, i.e. 10 g of glucose, can increase the osmotic pressure of 1 L of peritoneal dialysis fluid by 55 mOsm/kgH2O.

Adverse Reactions

(1) Phlebitis occurs when hypertonic glucose injection is dripped. If a large vein is used for dripping, the incidence of phlebitis decreases. (2) Extravasation of high-concentration glucose injection can cause local swelling and pain. (3) Reactive hypoglycemia: It is prone to occur when combined with excessive use of insulin, the original tendency to hypoglycemia, and when total intravenous nutrition therapy is suddenly stopped. (4) Hyperglycemic non-ketotic coma: It is more common in patients with diabetes, stress, the use of large amounts of glucocorticoids, and uremia peritoneal dialysis when hypertonic glucose solutions and total nutrition therapy are given intraperitoneally. (5) Electrolyte imbalance: Hypokalemia, hyponatremia, and hypophosphatemia are prone to occur when glucose is supplemented alone for a long time. (6) Patients with existing heart failure. (7) Hyperkalemia: It occasionally occurs in patients with type 1 diabetes when high-concentration glucose is used.
