Function and Efficacy

In vitro studies have shown that actinomycin D mainly acts on RNA, and at high concentrations affects both RNA and DNA synthesis. The mechanism of action is that it is embedded in the DNA double strand and binds to its guanine group, inhibiting the activity of DNA-dependent RNA polymerase, interfering with the transcription process of cells, and thus inhibiting mRNA synthesis. It is a non-specific drug for the cell cycle, and is particularly sensitive to the G1 phase, preventing G1 phase cells from entering the S phase.

Appearance

This product is a light orange-red crystalline powder, which is unstable when exposed to light.

Indication

1. It has outstanding efficacy in the treatment of Hodgkin's disease (HD) and neuroblastoma, especially in controlling fever. 2. The cure rate of this drug alone for the initial treatment of non-metastatic choriocarcinoma is 90% to 100%, which is similar to the effect of MTX alone. 3. It is also effective for testicular cancer, and is generally used in combination with other drugs. 4. Combining it with radiotherapy to treat childhood Wilms tumor can improve the survival rate, and it is also effective for Ewing sarcoma and rhabdomyosarcoma.

Usage and Dosage

Intravenous injection: 300-400 μg (6-8 μg/kg) per day for adults, dissolved in 20-40 ml of 0.9% sodium chloride injection, once a day, 10 days as a course of treatment, two weeks of rest, a total of 4-6 mg per course of treatment. This product can also be injected intracavitary. In combined chemotherapy, the dosage and time are not yet unified.

Adverse Reactions

1 Bone marrow suppression is a dose-limiting toxicity, with thrombocytopenia and granulocyte reduction, with the lowest value seen 10 to 21 days after administration, especially thrombocytopenia. 2 Gastrointestinal reactions are more common when each dose exceeds 500 mu; g, manifested as nausea, vomiting, diarrhea, and a few oral ulcers, which begin a few hours after medication and are sometimes severe, and are acute dose-limiting toxicity; 3 Hair loss begins 7 to 10 days after administration and is reversible. 4 A few cases of gastritis, enteritis, or skin erythema, desquamation, pigmentation, liver and kidney function damage, etc., are all reversible. 5 Leaking blood vessels cause significant damage to soft tissues.

Precautions

People with bleeding tendency should use this product with caution or not at all, and those with a history of chickenpox should not use it. This product has mutagenic, teratogenic and immunosuppressive effects, and is contraindicated for pregnant women.

Special Population Medication

Precautions for children: 0.45 mg/m2 per day, for five consecutive days, 3-6 weeks as a course of treatment. Use with caution in children under one year old. Precautions for pregnancy and lactation: Use with caution in lactating women. Because this product has mutagenic, teratogenic and immunosuppressive effects, it is contraindicated for pregnant women.

Drug Interactions

Vitamin K can reduce its potency, so vitamin K drugs should be used with caution when using this product; it has a radiosensitizing effect, but new inflammation may occur at the radiotherapy site, resulting in skin changes that recur after radiotherapy, which should be paid attention to.
