Donepezil Hydrochloride

Donepezil hydrochloride is a long-acting symptomatic treatment for Alzheimer's disease (AD). AD is an acquired intellectual impairment characterized by memory loss and accompanied by other cognitive impairments. Research in the past 30 years has shown that the progressive degeneration of cholinergic neurons in AD is the cause of memory loss, disorientation, behavioral and personality changes, and this cholinergic theory has been confirmed by histological studies. Donepezil hydrochloride is a second-generation cholinesterase (ChE) inhibitor, and its therapeutic effect is to reversibly inhibit the hydrolysis of acetylcholine caused by acetylcholinesterase (AchE) and increase the acetylcholine content at the receptor site. Donepezil may have other mechanisms, including the disposal of peptides, direct effects on neurotransmitter receptors or Ca2 channels.

Treatment of symptoms of mild or moderate Alzheimer's type dementia.

The most common symptoms are diarrhea, nausea, and insomnia, which are usually mild and transient and can be relieved within 1 to 2 days without stopping the drug.

It is contraindicated for patients with a history of allergy to donepezil hydrochloride, piperidine derivatives or excipients in the preparation; it is contraindicated for pregnant women.

Precautions during pregnancy and lactation: Contraindicated for use by pregnant women.

Donepezil hydrochloride is 96% bound to plasma proteins, but it does not replace other drugs that are highly bound to plasma proteins, such as furosemide, digoxin or warfarin, and these drugs do not replace its binding to human proteins. Studies have shown that donepezil hydrochloride does not affect the clearance of theophylline, cimetidine, warfarin or digoxin; it has a synergistic effect with cholinergic drugs and other inhibitors, and an antagonistic effect with anticholinergic drugs.
