Sodium Lactate Injection
Function and Efficacy
Under normal circumstances, the human body contains a small amount of lactic acid in the blood, which is mainly produced by glucose or glycogenolysis in tissues such as muscles, skin, brain and cells. After lactic acid is produced, it is either converted into glycogen or pyruvate, or enters the tricarboxylic acid cycle and is decomposed into water and carbon dioxide. Therefore, the final metabolite of sodium lactate is sodium bicarbonate, which can be used to correct metabolic acidosis. When hyperkalemia is accompanied by acidosis, sodium lactate can correct acidosis and allow potassium ions to enter cells from the blood and extracellular fluid. The main organs for lactic acid degradation are the liver and kidneys. When lactic acid metabolism in the body is abnormal or impaired, the therapeutic effect is poor; in addition, the effect of sodium lactate is not as rapid as that of sodium bicarbonate.

Indication
Used to correct metabolic acidosis, buffer in peritoneal dialysis fluid, hyperkalemia with severe arrhythmia and widened QRS wave.
Usage and Dosage
1. The dosage of metabolic acidosis is calculated according to the degree of acidosis. The volume (ml) of sodium lactate required for intravenous drip is the base deficit (mmol/L) × 0.3 × body weight (kg) = mol/L. Currently, sodium lactate is no longer used to correct metabolic acidosis. 2. Hyperkalemia can be treated with 40-60 ml of 11.2% injection solution intravenously for the first time, and then given as appropriate. Severe hyperkalemia leads to slow ectopic arrhythmias, especially when the QRS wave of the electrocardiogram is widened. The drug should be given under electrocardiogram monitoring. Sometimes up to 200 ml is required to be effective. At this time, attention should be paid to the blood sodium concentration and the prevention of heart failure. 3. Sodium lactate needs to be oxidized and metabolized into bicarbonate by the liver under aerobic conditions to play a role in correcting metabolic acidosis. Therefore, it is not as fast and stable as sodium bicarbonate and is now rarely used.
Adverse Reactions
1. People with hypocalcemia (such as uremia) are prone to numbness, pain, convulsions, and dyspnea after correcting acidosis, which is caused by the decrease in serum calcium ion concentration. 2. Accelerated heart rate, chest tightness, shortness of breath and other symptoms of pulmonary edema and heart failure. 3. Increased blood pressure. 4. Weight gain and edema. 5. Alkalosis occurs when the dosage is excessive. 6. Decreased blood potassium concentration, sometimes hypokalemia symptoms.
Precautions
1 Heart failure and acute pulmonary edema. 2 Brain edema. 3 Lactic acidosis is already significant. 4 Severe liver failure. 5 Severe renal failure with oliguria or anuria.
Drug Interactions
Sodium lactate is incompatible with neomycin sodium, tetracycline hydrochloride, and sulfadiazine sodium.
