Function and Efficacy

(1) Analgesic effect: It is mainly through inhibiting the synthesis of prostaglandins and other substances that can make pain sensitive to mechanical or chemical stimulation (such as bradykinin, histamine), and is a peripheral analgesic. However, the possibility of central analgesia (possibly acting on the hypothalamus) cannot be ruled out. (2) Anti-inflammatory effect: The exact mechanism is still unclear. It may be due to the fact that this product acts on inflamed tissues and inhibits the synthesis of prostaglandins or other substances that can cause inflammatory reactions (such as histamine) to exert an anti-inflammatory effect. Inhibition of the release of lysosomal enzymes and leukocyte chemotaxis may also be related to it. (3) Antipyretic effect: It may act on the hypothalamic temperature regulation center to cause peripheral vasodilation, increased skin blood flow, sweating, and increased heat dissipation to exert an antipyretic effect. This central effect may be related to the inhibition of prostaglandin synthesis in the hypothalamus. (4) Anti-rheumatic effect: The anti-rheumatic mechanism of this product, in addition to antipyretic and analgesic effects, is mainly anti-inflammatory. (5) Inhibit platelet aggregation: It works by inhibiting platelet cyclooxygenase and reducing the production of prostaglandins.

Appearance

This product is a capsule, the contents of which are enteric-coated pellets, which appear white after the enteric coating is removed.

Indication

This product can inhibit platelet aggregation and prevent thrombosis. It can be used to prevent thrombosis after transient ischemic attack, myocardial infarction, atrial fibrillation, artificial heart valve, arteriovenous fistula or other surgeries. It can also be used to treat unstable angina.

Usage and Dosage

Oral administration: Adults, 2-4 capsules at a time, 3 times a day.

Adverse Reactions

The doses generally used for antipyretic and analgesic rarely cause adverse reactions. Long-term and large-scale use of the drug (such as treatment of rheumatic fever), especially when the blood concentration of the drug is greater than 200μg/ml, is more likely to cause adverse reactions. The higher the blood concentration, the more obvious the adverse reactions. (1) The more common gastrointestinal reactions (incidence 3% to 9%) include nausea, vomiting, upper abdominal discomfort or pain (caused by the direct stimulation of the gastric mucosa by this product), which usually disappear after stopping the drug. Long-term or large-dose use may cause gastrointestinal bleeding or ulcers. (2) Central nervous system: reversible tinnitus and hearing loss occur, which usually occur after a certain course of treatment and the blood concentration reaches 200-300μg/L. (3) Allergic reaction: occurs in 0.2% of patients, manifested as asthma, urticaria, angioedema or shock. Most of them are susceptible people, and they quickly develop breathing difficulties after taking the drug. Severe cases can cause death, which is called aspirin asthma. Some are aspirin allergy, asthma and nasal polyps, which are often related to genetic and environmental factors. (4) Damage to liver and kidney function is related to the dosage, especially when the dosage is too large and the blood drug concentration reaches 250μg/ml. The damage is reversible and can be restored after stopping the drug. However, there are reports of renal papillary necrosis.

Precautions

(1) Patients who are allergic to this product are contraindicated. (2) Patients who are contraindicated in the following situations: ① Active ulcer disease or gastrointestinal bleeding caused by other reasons; ② Hemophilia or thrombocytopenia; ③ Patients with a history of allergy to aspirin or other non-steroidal anti-inflammatory drugs, especially those with asthma, angioedema or shock.

Special Population Medication

Precautions for children: Pediatric patients, especially those with fever and dehydration, are prone to toxic reactions. The use of this product in children with acute febrile diseases, especially influenza and chickenpox, may be associated with the development of Reye's syndrome, which is not common in China. Precautions for pregnancy and lactation: It is not clear. Precautions for the elderly: Elderly patients are prone to toxic reactions due to decreased renal function.

Drug Interactions

1. When used with other non-steroidal anti-inflammatory drugs, gastrointestinal adverse reactions increase, but the efficacy is not enhanced. 2. Antacids (long-term and large-scale use) or urine alkalinizing drugs: urine alkalinization can increase the excretion of this product and reduce the blood concentration. 3. When used with oral anticoagulants, the risk of bleeding may increase. 4. When used with other salicylates, the plasma concentration of salicylate may increase and even cause toxic reactions. 5. Carbonic anhydrase inhibitors can alkalinize urine, which can not only increase the excretion of salicylate and reduce the blood concentration, but also increase the amount of metabolic acidosis that penetrates into the brain tissue and cause toxic reactions. 6. Glucocorticoids can increase the excretion of salicylates. In order to maintain the blood concentration of this product when used together, the dosage of this product should be increased if necessary. 7. The efficacy of insulin or oral hypoglycemic drugs can be more obvious when used with large amounts of salicylates. 8. When used with methotrexate, it can reduce the binding of methotrexate to proteins, reduce its excretion from the kidneys, increase blood concentrations and increase toxic reactions. 9. Uric acid drugs: Acidic urine can reduce the excretion of salicylates, increasing the latter's blood concentration. Patients whose blood salicylic acid concentration has reached a stable state may increase their blood salicylate concentration and increase toxic reactions after adding uricidifying drugs.
