Midodrine Hydrochloride Tablets

Function and Efficacy

The original form of this product is inactive. After oral administration, it is converted into the active metabolite desglycine midodrine after aminoacetyl hydrolysis in the blood. Desglycine midodrine is a selective peripheral sympathetic nerve a1 agonist, directly acting on the postsynapse to release adrenergic stimulants. This product has the function of regulating vascular tension. It can increase peripheral arterial and venous resistance for patients with orthostatic blood pressure regulation abnormalities, prevent a large amount of blood from accumulating in the lower limbs, promote blood return, and keep blood volume stable, thereby increasing the patient's orthostatic blood pressure and improving symptoms caused by insufficient circulation volume (such as lack of energy, fatigue, dizziness, blurred vision, etc. in the morning). This product will not cause increased excitability of the central nervous system and has no direct effect on the heart, but the heart rate may decrease due to feedback after medication.

Indication

It is used for hypotension caused by postural dysfunction of blood circulation when the veins of the lower limbs are congested, postoperative blood loss, postpartum blood loss, climate change, and hypotension caused by fatigue after getting up in the morning. Female stress urinary incontinence.

Usage and Dosage

Treatment is carried out and adjusted accordingly according to the patient's autonomic nervous system tension and responsiveness. The following dosage is recommended: 1. Adults and adolescents (over 12 years old): Starting dose 2.5 mg (1 tablet) 2-3 times a day. The above dosage can be increased every 3-4 days depending on the patient's tolerance to this drug. Midodrine hydrochloride tablets should be taken during the day when the patient needs to stand up to carry out daily activities. It is recommended that the interval between medications be 3-4 hours. The first dose should be taken before or after getting up in the morning, the second dose should be taken at noon, and the third dose should be taken in the afternoon or late at night. To prevent supine hypertension, midodrine hydrochloride tablets should not be taken within 4 hours after dinner or before bedtime. 2. Precautions

Patients with hypertension, pheochromocytoma, acute nephritis, severe renal dysfunction, glaucoma, prostatic hyperplasia with urinary retention, mechanical urination obstruction, and hyperthyroidism are prohibited from using this medicine. Patients with severe organic heart disease, cardiovascular disease or arrhythmia and renal insufficiency should pay attention to the dosage when using the medicine.

Special Population Medication

Precautions for children: Not yet clear. Precautions for pregnancy and lactation: Use with caution for pregnant women. Precautions for the elderly: Not yet clear.

Drug Interactions

Concomitant use with sympathomimetics or other vasoconstrictors such as reserpine, guanethidine, tricyclic antidepressants, antihistamines, thyroid hormones, and MAO inhibitors should be avoided because a significant increase in blood pressure may occur. The effect of midodrine hydrochloride may be antagonized by alpha-receptor blockers (such as prazosin and phentolamine). Combination with beta-receptor blockers may lead to an increase in bradycardia. Rhodiola alkaloids (reserpine) can slightly increase the pressor effect of midodrine hydrochloride. Cardiac glycosides (digitalis) can cause an increase in bradycardia or cardiac conduction disorders. The use of atropine or cortisone drugs during the week can increase or excessively increase blood pressure.
