Nicardipine Hydrochloride Injection
Function and Efficacy
This product inhibits the transmembrane calcium ion influx of myocardial and vascular smooth muscle without changing the blood calcium concentration. Its effect on vascular smooth muscle is better than that on myocardial, so its vascular selectivity is relatively strong. In animal experiments, this product selectively dilates coronary vascular smooth muscle. The blood drug concentration when this effect occurs does not produce negative inotropic effect (myocardial), and has little effect on heart rhythm and cardiac contractility. In the human body, this product reduces peripheral vascular resistance. This effect is greater in patients with hypertension than in those with normal blood pressure. When blood pressure is lowered, there will be a reflex increase in heart rate. This product increases cardiac ejection fraction and cardiac output, while the left ventricular end-diastolic pressure does not change much. It can reduce myocardial oxygen consumption and total peripheral resistance, and can also increase coronary collateral circulation and increase coronary blood flow. This product is a dihydropyridine calcium antagonist that can block the influx of calcium ions into vascular smooth muscle cells, thereby dilating blood vessels and lowering blood pressure.

Indication
Emergency treatment for abnormally high blood pressure during surgery; hypertensive emergency.
Usage and Dosage
Emergency treatment of abnormally high blood pressure during surgery: After diluting this product with normal saline or 5% glucose injection, the solution of 0.01-0.02% (0.1-0.2 mg in 1 ml) of nicardipine hydrochloride is intravenously dripped. At this time, start the dripping at a rate of 2-10 μg/kg (body weight) per minute. After the blood pressure drops to the target value, adjust the dripping rate while monitoring the blood pressure. If it is necessary to quickly lower the blood pressure, this product is intravenously administered at a dose of 10-30 μg/kg (body weight) of nicardipine hydrochloride.
Adverse Reactions
1. The circulatory system may sometimes experience tachycardia, palpitations, flushed face, general discomfort, and electrocardiogram changes. 2. The liver may sometimes experience liver dysfunction (increase in GOT, GPT, etc.). 3. The kidneys may sometimes experience increase in BUN and creatinine. 4. The digestive system may sometimes experience nausea. 5. Others may sometimes experience hypoxemia, headache, increased body temperature, decreased urine volume, and decreased total blood cholesterol.
Precautions
1. Patients with intracranial hemorrhage, and the bleeding is estimated to have not yet been completely stopped. 2. Patients with increased intracranial pressure in the acute stage of cerebral stroke. 3. Patients with a history of allergy to nicardipine hydrochloride.
Drug Interactions
1. Antihypertensive drugs (will intensify the effects of antihypertensive drugs); 2. beta-blockers (patients with congestive heart failure sometimes show negative inotropic effects); 3. cimididine will increase the blood concentration of this product; 4. digoxin (will increase the blood concentration of digoxin); 5. fentanyl anesthesia (when used in combination with beta-blockers, hypotension may sometimes occur); 6. cyclosporine (will increase the blood concentration of cyclosporine); 7. phenytoin (will increase the blood concentration of phenytoin and cause symptoms of neurotoxicity); 8. nitrofurantoin (there are reports that ventricular fibrillation has been observed in animal experiments using other calcium antagonists); 9. nitroglycerin (there are reports that atrioventricular conduction block has occurred).
