Function and Efficacy

This drug contains two active ingredients, namely isosorbide mononitrate and aspirin. Isosorbide mononitrate is the main active metabolite of isosorbide dinitrate. It can reduce myocardial oxygen consumption by dilating peripheral blood vessels, especially increasing venous blood volume, reducing venous return, and reducing cardiac preload and postload; it can also promote myocardial blood flow redistribution and improve blood supply to ischemic areas. Isosorbide mononitrate may play an anti-myocardial ischemic role through these two aspects. Aspirin acetylates platelet cyclooxygenase (i.e., prostaglandin synthase), thereby reducing the generation of thromboxane A2 (TXA2), producing an irreversible inhibitory effect on TXA2-induced platelet aggregation, and also inhibiting ADP or adrenaline-induced phase II aggregation; it can also inhibit platelet aggregation and release reactions and spontaneous aggregation caused by low concentrations of collagen, thrombin, antibody-antigen complexes, certain viruses and bacteria, thereby preventing the formation of thrombus. At high concentrations, aspirin can also inhibit PG synthase in the blood vessel wall and reduce the synthesis of prostacyclin (PGI2). PGI2 is a physiological antagonist of TXA2, and its reduced synthesis may promote the formation of blood clots.

Appearance

This product is a film-coated tablet. After removing the film coating, it becomes a white or off-white double-layer tablet.

Indication

Angina pectoris, used to prevent recurrence of angina pectoris after the initial attack.

Usage and Dosage

Oral administration: swallow the whole tablet without chewing it. Take it at the same time every morning. The starting dose is 1 tablet per day. Adjust the dose according to the doctor's advice. Consult a doctor when stopping the medication. Do not increase the dose after missing a dose.

Adverse Reactions

1. Side effects such as skin flushing (a manifestation of vasodilation) are generally mild. 2. Allergic reactions, fatigue, nausea, vomiting, etc. are occasionally seen, and generally disappear on their own after stopping the drug.

Precautions

Patients with the following symptoms are contraindicated: 1. Allergy to the active ingredients of this product or similar drugs (non-steroidal anti-inflammatory drugs, NSAIDS); 2. Asthma; 3. Cerebral hemorrhage; 4. Bleeding and coagulation disorders; 5. Children under 16 years old; 6. Craniocerebral injury; 7. Heart attack caused by other cardiac reasons; 8. Hypotension; 9. Decreased blood volume; 10. Peptic ulcer; 11. Severe anemia; 12. Severe cerebrovascular insufficiency; 13. Acute myocardial infarction with decreased left ventricular filling pressure; 14. Evidence has shown that sildenafil can enhance the antihypertensive response of nitrates, so the combination of the two is contraindicated.

Special Population Medication

Precautions for children: Children under 16 years old are prohibited from taking this product. Precautions for pregnancy and lactation: Pregnant women should use this product with caution in the early and mid-pregnancy period, because this product can increase the risk of complications for mothers and newborns during delivery, so women in the first and last three months of pregnancy should not use this product. This product and its degradation products can be secreted through breast milk, so lactating women should use it with caution. When taking large doses (more than 150 mg of aspirin per day), breastfeeding should be stopped. Precautions for the elderly: Elderly patients may be more sensitive to this type of drug and are more likely to experience dizziness and other reactions.

Drug Interactions

This drug is not suitable for use with sildenafil because it can cause a sharp drop in blood pressure, resulting in dizziness and syncope. Use with sildenafil may also reduce the amount of blood supplied to the heart, leading to a heart attack. Aspirin enhances the effects of the following drugs: · Anticoagulants (such as coumarin derivatives, heparin); · Risk of gastrointestinal bleeding caused by concurrent use of drugs containing cortisone or cortisone analogs, or alcohol; · Certain hypoglycemic drugs (sulfonylureas); · Methotrexate; · Digoxin, barbiturates and lithium; · Certain analgesics, anti-inflammatory drugs and anti-rheumatic drugs (non-steroidal anti-inflammatory analgesics) and general anti-rheumatic drugs; · Certain antibiotics (sulfonamides and sulfonamide complexes such as sulfamethoxazole/trimethoprim); · Triiodothyronine. Aspirin can reduce the elimination rate of methotrexate in the body, significantly increasing the effects of methotrexate and enhancing its toxicity, so these two drugs are usually not suitable for use together. This product should not be taken within 8-12 days of taking mifepristone, because aspirin will reduce the efficacy of mifepristone. When using low-dose aspirin for anticoagulation, it should not be used in combination with any other non-steroidal anti-inflammatory drugs, because this may cause gastric ulcers and bleeding. When aspirin is used in combination with corticosteroids, there is also a risk of gastric ulcers and bleeding. Aspirin can reduce the antihypertensive effect of certain antihypertensive drugs, such as enalapril. Aspirin can also reduce the therapeutic effect of diuretics such as spironolactone and thiazide drugs. Isosorbide mononitrate can enhance the antihypertensive effect of antihypertensive drugs. If used simultaneously with other vasodilators, calcium antagonists, antihypertensive drugs, tricyclic antidepressants and alcoholic liquids, attention should be paid to the effect on blood pressure.
