Function and Efficacy
1 Pharmacology Pingyangmycin is a bleomycin antitumor antibiotic produced by Streptomyces Pingyangensis n.S.P. It can inhibit the synthesis of cancer cell DNA and cut off the DNA chain, affect the metabolic function of cancer cells, and promote the degeneration and necrosis of cancer cells. In vitro experiments have shown that Pingyangmycin has a strong killing effect on a variety of cultured cancer cells such as liver cancer BEL7402, gastric cancer MGC803, nasopharyngeal cancer CNB-2, colon cancer HT-29 and oral squamous cell carcinoma KB. In vivo: Pingyangmycin has a significant inhibitory effect on rat subcutaneously inoculated Wacker tumor 256, mouse sarcoma 180, sarcoma 37, liver cancer and melanoma, and has a stronger anti-tumor effect on mouse subcutaneously inoculated colon cancer C20, esophageal cancer SGA73 and Lewis lung cancer than mitomycin and Bleomycin A2. Pingyangmycin is a non-specific drug for the cell cycle and has no significant effect on the body's immune function and hematopoietic function. 2 Toxicology 18-20g mice, half male and half female, were randomly divided into groups, 10 animals in each group, injected intravenously or intraperitoneally with pingyangyunmycin, observed for 14 days, recorded the number of dead animals, and determined the LD50 and LD10 by probability logarithm plotting method. Results: The LD50 of intravenous injection of pingyangyunmycin was 165ml/Kg, and the LD10 was 100ml/Kg; the LD50 of intraperitoneal injection was 188ml/Kg, and the LD10 was 125ml/Kg.

Appearance
This product is a white loose mass or amorphous solid, odorless or almost odorless, and highly hygroscopic. It is easily soluble in water or methanol, slightly soluble in ethanol, and almost insoluble in acetone, chloroform or ether.
Indication
It is mainly used to treat lip cancer, tongue cancer, gum cancer, nasopharyngeal cancer and other cervical squamous cell carcinomas.
Usage and Dosage
1 Intravenous injection: Dissolve this product in 5-20 ml of a suitable intravenous injection solution such as normal saline or glucose solution to a concentration of 4-15 mg (titer)/ml and inject. 2 Intramuscular injection: Dissolve this product in less than 5 ml of normal saline to a concentration of 4-15 mg (titer)/ml and inject. 3 Intraarterial injection: Dissolve this product in 3-25 ml of normal saline with an anticoagulant (such as heparin) to 4-8 mg (titer) for a single intraarterial injection or continuous intraarterial injection. 4 The dose for adults is 8 mg (titer) per time, usually 2-3 times a week. It can be increased or decreased to once a day or once a week depending on the patient's condition. The dose that shows efficacy is generally 80-160 mg (titer). The total dose for a course of treatment is 240 mg (titer). 5 After the tumor disappears, the medication should be appropriately increased, such as 8 mg (potency) intravenous injection once a week for about 10 times. 6 Treatment of hemangioma and lymphangioma: Intratumoral injection of bleomycin for the treatment of lymphangioma: 4-8 mg each time, dissolved in 2ml-4ml of injection water, and the cystic fluid should be drained as much as possible before injection in patients with cysts. The interval period is at least 1 month, and 5 times is a course of treatment. It should not be used for newborns under 3 months old or should be used in reduced doses. Treatment of hemangioma: 4-8 mg of bleomycin is injected each time, with normal saline or lidocaine Dilute 3ml~5ml of injection solution and inject it into the tumor. If the patient is not cured after one injection, repeat the injection after 7~10 days. The total amount of drug should not exceed 70mg (potency).7Treatment of nasal polyps: Take 1 vial of pingyangmycin (containing 8mg) and dissolve it in 4ml of normal saline. Use a thin needle to inject it into the polyp. Each time, 2ml~4ml is injected into the polyp, that is, 1~2 polyps are injected at a time. Observe for 15~30 minutes for allergic reactions. Do it once a week. Five times constitute a course of treatment. Generally, 1~2 courses of treatment are required.
Adverse Reactions
The main adverse reactions of bleomycin include fever, gastrointestinal reactions (nausea, vomiting, loss of appetite, etc.), skin reactions (pigmentation, thickening of keratinization, dermatitis, rash, etc.), hair loss, acroparesis and oral inflammation, etc. The incidence of lung symptoms (pneumonia-like lesions or pulmonary fibrosis) is lower than that of bleomycin.
Precautions
1. Patients with a history of allergy to bleomycin antibiotics are contraindicated. 2. Patients with lung, liver, and kidney dysfunction should use with caution.
