Function and Efficacy
Pefloxacin mesylate is a quinolone antibacterial drug, and its mechanism of action is to inhibit bacterial DNA helicase. It has a broad-spectrum antibacterial effect, and has strong antibacterial activity against Enterobacter bacteria such as Escherichia coli, Klebsiella, Proteus, Shigella, Salmonella typhi, etc., as well as influenza bacillus, Neisseria, etc. It also has a certain antibacterial effect on Staphylococcus aureus and Pseudomonas aeruginosa. It has only a mild effect on pneumococci, various groups of streptococci and enterococci.

Indication
Various infections caused by pefloxacin-sensitive bacteria: urinary tract infections; respiratory tract infections; ear, nose and throat infections; gynecological and reproductive system infections; abdominal and liver and gallbladder system infections; bone and joint infections; skin infections; sepsis and endocarditis; meningitis.
Usage and Dosage
Intravenous drip: Common adult dosage, 0.4g at a time, add to 250ml of 5% glucose solution and slowly drip intravenously, once every 12 hours. Patients with jaundice should take the medicine once a day; patients with ascites should take the medicine once every 36 hours; patients with jaundice and ascites should take the medicine once every 48 hours. Adverse Reactions
1. Gastrointestinal reactions: nausea, vomiting, loss of appetite, diarrhea, etc. 2. Central nervous system reactions: dizziness, headache, drowsiness or insomnia, etc. 3. Allergic reactions: rash, skin itching, occasionally exudative, multiforme erythema and angioneurotic edema. A few patients have photosensitivity reactions, etc. 4. Occasionally may occur: ① epileptic seizures, mental abnormalities, irritability, confusion, hallucinations, tremors; ② interstitial nephritis manifestations such as hematuria, fever, rash; ③ crystalluria, more common when high doses are used; ④ joint pain; ⑤ local irritation symptoms at the injection site. 5. A small number of patients may experience elevated serum aminotransferase, increased blood urea nitrogen and decreased peripheral white blood cells, which are mostly mild and transient.
Precautions
It is contraindicated for patients who are allergic to pefloxacin mesylate and fluoroquinolones, pregnant and lactating women, patients under 18 years of age, and patients with 6-phosphate glucose dehydrogenase deficiency.
Drug Interactions
1. Urine alkalinizing agents can reduce the solubility of this product in urine, leading to crystalluria and nephrotoxicity. 2. When this product is used in combination with theophylline, competitive inhibition of the binding site of cytochrome P450 may lead to a significant reduction in the elimination of theophylline in the liver, a prolonged plasma elimination half-life (t1/2?), an increase in blood drug concentration, and the occurrence of symptoms of theophylline poisoning, such as nausea, vomiting, tremor, restlessness, excitement, convulsions, palpitations, etc., so it should be avoided. If it cannot be avoided, theophylline blood drug concentration should be measured and the dose adjusted. 3. When cyclosporine is used in combination with this product, its blood drug concentration increases. The blood concentration of cyclosporine must be monitored and the dose adjusted. 4. When this product is used in combination with the anticoagulant warfarin, it can enhance the latter's anticoagulant effect. When used in combination, the patient's prothrombin time should be closely monitored and the dose adjusted. 5. Probenecid can reduce the secretion of this product from the renal tubules by 50%. When used in combination, it may cause toxicity due to the increased blood concentration of this product. 6. This product interferes with the metabolism of caffeine, resulting in reduced caffeine elimination, prolonged blood elimination half-life (t1/2?), and possible central nervous system toxicity. When used together, the patient's blood caffeine concentration should be closely monitored and the dosage adjusted.
