Function and Efficacy

There are various natural antagonists of plasminogen in the blood circulation, such as antiplasmin. Under normal circumstances, the antifibrinolytic activity in the blood is many times higher than the fibrinolytic activity, so fibrinolytic bleeding does not occur. However, these antagonists cannot block the activation of activators (such as urokinase, etc.) adsorbed on the fibrin network to form plasmin. Plasmin is a peptide chain endonuclease that can cleave the arginine and lysine peptide chains of fibrinogen in a neutral environment to form fibrin degradation products and cause blood clots to dissolve and bleed. Plasminogen specifically adsorbs on fibrin through the lysine binding site in its molecular structure, and lysine can competitively inhibit this adsorption, reduce the adsorption rate of plasminogen, and thus reduce the degree of plasminogen activation to reduce bleeding. The chemical structure of this product is similar to lysine (1,5-diaminohexanoic acid), so it can also competitively inhibit the adsorption of plasminogen on fibrin, thereby preventing its activation and protecting fibrin from being degraded and dissolved by plasmin, ultimately achieving a hemostatic effect. This product can also directly inhibit the activity of plasmin and reduce the role of plasmin in activating complement, thereby preventing the occurrence of hereditary angioedema.

Indication

1. This product is mainly used for various bleeding caused by acute or chronic, localized or systemic primary hyperfibrinolysis. Secondary hyperfibrinolytic state caused by disseminated intravascular coagulation is generally not used before heparinization; 2. It is used for trauma or surgical bleeding in organs rich in plasminogen activators such as the prostate, urethra, lungs, brain, uterus, adrenal glands, and thyroid glands; 3. It is used as an antagonist of tissue plasminogen activator (t-PA), streptokinase, and urokinase; 4. It is used for fibrinolytic bleeding caused by artificial abortion, early placental abruption, stillbirth, and amniotic fluid embolism, as well as menorrhagia caused by pathological increased local fibrinolysis in the uterine cavity; 5. Usage and Dosage

Intravenous injection or drip: 0.25-0.5g once, 0.75-2g per day. The intravenous injection solution is diluted with 25% glucose solution, and the intravenous drip solution is diluted with 5%-10% glucose solution. To prevent bleeding before and after surgery, the above dosage can be used as a reference. When treating bleeding caused by primary fibrinolysis, the dosage can be increased as appropriate.

Adverse Reactions

This product has fewer adverse reactions than 6-aminocaproic acid: 1. Occasionally, drug overdose may cause intracranial thrombosis and bleeding; 2. Diarrhea, nausea and vomiting may occur; 3. Less common is menstrual discomfort (caused by blood coagulation during menstruation); 4. Because this product can enter the cerebrospinal fluid, after injection, there may be blurred vision, headache, dizziness, fatigue and other central nervous system symptoms, which are particularly related to the injection speed, but are rare.

Precautions

1. Use with caution in patients with thrombotic tendency (such as acute myocardial infarction); 2. Because this product can cause secondary pyelonephritis and ureteral clot obstruction, it should be used with caution in patients with hemophilia or renal pelvic parenchymal lesions with massive hematuria.

Drug Interactions

The combined use of oral contraceptives, estrogens or prothrombin complex concentrates with this product may increase the risk of thrombosis.
