Mexiletine Hydrochloride Injection

Function and Efficacy

It is a Class Ib antiarrhythmic drug that can inhibit sodium influx into myocardial cells, reduce the speed of phase 0 depolarization of action potentials, and shorten the effective refractory period of Purkinje fibers. In patients with normal cardiac conduction systems, mexiletine has little effect on the generation and conduction of cardiac impulses. Clinical trials have not found that mexiletine causes II or III degree atrioventricular block. Mexiletine does not prolong the duration of ventricular depolarization and repolarization, so it can be used for ventricular arrhythmias with prolonged QT intervals. The drug has antiarrhythmic, anticonvulsant and local anesthetic effects. It has a small inhibitory effect on the myocardium. The effective blood concentration of mexiletine is 0.5-2ug/ml, and the toxic blood concentration is close to the effective blood concentration, which is above 2ug/ml. A small number of patients may experience serious adverse reactions at the effective blood concentration.

Appearance

This product is a colorless clear liquid.

Indication

It is mainly used for acute ventricular arrhythmias, such as sustained ventricular tachycardia. It should be avoided for asymptomatic ventricular premature beats.

Usage and Dosage

Intravenous injection: Start with 100 mg, add to 20 ml of 5% glucose solution, and slowly inject intravenously for 3 to 5 minutes. If ineffective, give 50 to 100 mg again after 5 to 10 minutes. Then drip at a rate of 1.5 to 2 mg/min for 3 to 4 hours, then reduce the drip rate to 0.75 to 1 mg/min and maintain for 24 to 48 hours.

Adverse Reactions

There may be nausea, vomiting, drowsiness, bradycardia, hypotension, tremor, headache, dizziness, etc. Large doses may cause hypotension, bradycardia, conduction block, etc.

Precautions

It is contraindicated in patients with cardiogenic shock, II or III degree atrioventricular block, and sick sinus syndrome.

Drug Interactions

Clinical trials have reported that there is no mutual effect between mexiletine and commonly used anti-anginal, antihypertensive and antifibrinolytic drugs. If phenytoin sodium or other liver enzyme inducers such as rifampicin and phenobarbital are used in combination with mexiletine, the blood concentration of mexiletine can be reduced. It has been reported that benzodiazepines do not affect the blood concentration of mexiletine. The combination of mexiletine and digoxin, diuretics and propranolol does not affect the PR, QRS and QT intervals of the electrocardiogram.
