Function and Efficacy

This product is a synthetic nucleoside antiviral drug that has inhibitory effects on herpes simplex virus type I (HSV-1), type H (HSV-2) and varicella-zoster virus (VZV) both in vivo and in vitro. Cell culture results show that this product has the strongest inhibitory effect on HSV-1 virus, followed by HSV-2 and VZV viruses. Because this product has affinity for thymidine kinase (TK) encoded by HSV and VZV, it has a highly selective inhibitory effect. This type of viral enzyme converts acyclovir into acyclovir monophosphate, a nucleoside analog. The monophosphate is further converted into diphosphate by guanylate kinase in the cell, and then converted into triphosphate by multiple enzymes in the cell. In vitro, acyclovir triphosphate stops herpes virus DNA replication.

Appearance

This product is a colorless clear liquid.

Indication

This product is used for 1. Herpes simplex virus infection: for the treatment of primary and recurrent mucocutaneous infections in immunocompromised individuals and the prevention of recurrent cases; also for the treatment of herpes simplex encephalitis. 2. Herpes zoster: for the treatment of severe herpes zoster in immunocompromised individuals or diffuse herpes zoster in immunocompetent individuals. 3. The treatment of varicella in immunocompromised individuals.

Usage and Dosage

For intravenous infusion only, and each infusion time is more than 1 hour. Common adult dosage: 1. For the initial treatment of severe genital herpes, 5 mg/kg (calculated as acyclovir, the same below) per body weight, 3 times a day, drip every 8 hours, for 5 days. 2. For immunocompromised patients with skin and mucous membrane herpes simplex or severe herpes zoster, 5-10 mg/kg per body weight, 3 times a day, drip every 8 hours, for 7-10 days. 3. For herpes simplex encephalitis, 10 mg/kg per body weight, 3 times a day, drip every 8 hours, for 10 days. The maximum daily dose for adults is 30 mg/kg per body weight, or 1.5 g/m2 per body surface area.

Adverse Reactions

1 Common adverse reactions: Inflammation or phlebitis at the injection site, itching or urticaria, rash, fever, mild headache, nausea, vomiting, diarrhea, proteinuria, increased blood urea nitrogen and serum creatinine, abnormal liver function such as mild increase in serum aminotransferase, alkaline phosphatase, lactate dehydrogenase, and total bilirubin. 2 Rare adverse reactions include: acute renal insufficiency, decreased white blood cells and red blood cells, decreased hemoglobin, neutropenia, thrombocytopenic purpura, increased cholesterol and triglycerides, hematuria, hypotension, sweating, palpitations, dyspnea, and chest tightness. 3 Adverse reactions observed in clinical practice after the product was marketed include: (1) Digestive system:

Precautions

It is contraindicated in patients who are allergic to acyclovir.

Special Population Medication

Precautions for children: Although no special adverse reactions have been found in children, it should be used with caution. It is not advisable for newborns to prepare infusion solutions with diluents containing benzyl alcohol, otherwise it is easy to cause fatal syndromes, including acidosis, central nervous system depression, dyspnea, renal failure, hypotension, epilepsy and intracranial hemorrhage. Precautions for pregnancy and lactation: Use with caution in pregnant women. From 1984 to April 1994, an epidemiological record on the efficacy of acyclovir in pregnant women was established. Among the results of 756 experiments over a period of 3 months, 749 pregnant women successively took acyclovir systematically, and its teratogenic effect on infants was similar to that of the general population. However, these data are not enough to prove that acyclovir is completely safe for pregnant women and fetuses. Acyclovir can only be considered when the therapeutic effect of acyclovir on the fetus is far greater than its risk. The concentration of acyclovir in breast milk is 0.6 to 4.1 times the blood concentration. When the dosage of a lactating woman reaches 0.3 mg/kg per day, it may affect the development of the baby. Acyclovir can only be used by breastfeeding women when necessary. Precautions for the elderly: There is currently no sufficient research data to show that the use of drugs for the elderly over 65 years old is significantly different from that for young people. In general, the elderly should be cautious when using drugs, choose a low dose of effective medication, and try to reduce the occurrence of renal function impairment or other side effects caused by increasing the number of medications.

Drug Interactions

1. When used in combination with interferon or methotrexate (intrathecal), it may cause mental abnormalities and should be used with caution. 2. When used in combination with nephrotoxic drugs, it can aggravate nephrotoxicity, especially in patients with renal insufficiency. 3. When used in combination with zidovudine, it can cause nephrotoxicity, manifested as severe lethargy and fatigue. 4. It competitively inhibits the secretion of organic acids with probenecid. When used in combination with probenecid, it can slow down the excretion of this product, prolong its half-life, and cause drug accumulation in the body.
