Function and Efficacy

1. This product is a dihydropyridine calcium channel blocker. 2. This product inhibits the transmembrane calcium influx of vascular smooth muscle and myocardium, but mainly acts on blood vessels, so its vascular selectivity is relatively strong. 3. This product causes the dilation of systemic blood vessels such as coronary arteries and renal arterioles, producing a hypotensive effect. Carcinogenicity, mutagenicity and reproductive toxicity Rats taking the same drug nifedipine orally for two years did not show carcinogenicity. The results of in vivo mutagenicity studies were negative.

Usage and Dosage

Common dosage for adults: Initially take 10 mg orally once a day, which can be adjusted to 20 mg twice a day depending on the situation.

Adverse Reactions

Less common symptoms include headache and facial flushing. Less common symptoms include dizziness, nausea, hypotension, ankle edema, angina attack, and one-time hypotension. People who are allergic to this product may develop allergic hepatitis, rash, and even exfoliative dermatitis.

Precautions

It is contraindicated for patients who are allergic to this product or have severe aortic stenosis.

Special Population Medication

Precautions for children: Not clear yet. Precautions for pregnancy and lactation: The research on the use of this product in pregnant women is not sufficient, and there have been no problems in the existing clinical application, but adverse reactions should be noted. Precautions for the elderly: The blood concentration of the drug is higher in the elderly, but the half-life is not prolonged, so the dose should be appropriately reduced; those who are taking β-receptor blockers should be cautious when adding this product. It is advisable to start with a small dose when combined with other drugs to prevent inducing or aggravating systemic hypotension, increasing the occurrence of angina pectoris, heart failure, and even myocardial infarction. The recommended initial dose for elderly patients is 10 mg per day.

Drug Interactions

1. Most patients with y-receptor blockers can enhance the antihypertensive effect by taking this drug together, and can reduce the tachycardia that occurs after the drug lowers blood pressure; however, some patients may induce and aggravate systemic hypotension, heart failure and angina pectoris. 2. The co-administration of angiotensin-converting enzyme inhibitors is well tolerated and the antihypertensive effect is enhanced. 3. The co-administration of long-acting nitrates is well tolerated, but there is still a lack of literature evaluating the effectiveness of this combination in controlling angina pectoris. 4. Some studies on digitalis suggest that taking this drug can increase the plasma concentration of digoxin in combination, with an average increase of 45%. Some studies believe that it does not increase the plasma concentration and toxicity of digoxin. It is suggested that we should monitor the blood concentration of digoxin when using it for the first time, adjusting the dose or stopping the use of nitrendipine to prevent digoxin overdose or underdose. 5. There are no reports that the co-administration of nitrendipine can increase the prothrombin time of coumarin anticoagulants. At present, the interaction between them cannot be confirmed. 6. Cimetidine: Since cimetidine can mediate the inhibition of liver cytochrome P450 enzymes and change the first-pass effect of nitrendipine, it is recommended that patients who are taking cimetidine should pay attention to the adjustment of drug dosage when taking nitrendipine in combination.
