Levothyroxine Sodium is the sodium salt of levothyroxine, a synthetic levoisomer of thyroxine (T4) that is similar to the endogenous hormone produced by the thyroid gland. In peripheral tissues, levothyroxine is deiodinated by 5'-deiodinase to form triiodothyronine (T3). T3 enters the cell and binds to nuclear thyroid hormone receptors; the activated hormone-receptor complex in turn triggers gene expression and produces proteins required in the regulation of cellular respiration; thermogenesis; cellular growth and differentiation; and the metabolism of proteins, carbohydrates and lipids. T3 also exhibits cardiostimulatory effects.|The major hormone derived from the thyroid gland. Thyroxine is synthesized via the iodination of tyrosines (MONOIODOTYROSINE) and the coupling of iodotyrosines (DIIODOTYROSINE) in the THYROGLOBULIN. Thyroxine is released from thyroglobulin by proteolysis and secreted into the blood. Thyroxine is peripherally deiodinated to form TRIIODOTHYRONINE which exerts a broad spectrum of stimulatory effects on cell metabolism.
On August 14, 1997, the FDA declared levothyroxine sodium tablets a “new drug.” On February 15, 2017, the FDA approved levothyroxine sodium (brand name: Tirosint-SOL, developed by IBSA Pharma Inc.) oral solution for hypothyroidism and pituitary thyroid-stimulating hormone (TSH) suppression. Subsequently, the FDA approved two changes to the labeling of levothyroxine sodium (Tirosint-SOL, IBSA). The first change was for use in combination with a proton pump inhibitor (PPI). The second adjustment was the timing of levothyroxine sodium administration. The approved labeling changes were based on the results of two clinical studies that found no interaction between levothyroxine sodium and PPIs, and the same results were shown when administered 15 minutes and 30 minutes before breakfast. This led to the approval to label levothyroxine sodium as the only FDA-approved LT4 therapy that does not have label interactions with PPIs and can be taken 15 minutes before breakfast.
