Amantadine hydrochloride is an antiviral and antiparkinsonian medication that is primarily used in the treatment of influenza A and for managing symptoms of Parkinson's disease.

Mechanism of Action

Antiviral Activity: Amantadine interferes with the viral replication of influenza A by disrupting the function of the M2 protein, which is essential for the uncoating process of the virus. This action prevents the release of viral nucleic acids into host cells, thereby inhibiting infection. It has minimal or no efficacy against influenza B due to differences in the M2 protein structure.
Antiparkinsonian Effects: In treating Parkinson's disease, amantadine is thought to enhance dopaminergic activity in the brain. It acts as a weak, non-competitive antagonist of the NMDA receptor, which can lead to increased dopamine release and reduced reuptake. Although it does not have direct anticholinergic activity, it may produce anticholinergic-like side effects.

Metabolism: Minimal metabolism occurs; however, N-acetylamantadine is a principal metabolite. Amantadine undergoes N-acetylation and N-methylation.
Elimination: Primarily excreted unchanged via urine through glomerular filtration and tubular secretion. The elimination half-life ranges from 10 to 14 hours but can extend significantly in patients with renal impairment (up to 7–10 days).
