Clindamycin is a lincosamide antibiotic widely used for treating various bacterial infections.

Mechanism of Action

Clindamycin inhibits bacterial protein synthesis by binding to the 50S subunit of the bacterial ribosome, specifically to the 23S rRNA. This binding interferes with ribosomal translocation and peptide bond formation, leading to a bacteriostatic effect, although at higher concentrations it can be bactericidal . Clindamycin's action is similar to that of macrolides and streptogramins, which share overlapping binding sites on the ribosome . Resistance to clindamycin can occur due to mutations in the rRNA or through cross-resistance with other antibiotics in its class

Metabolism
Clindamycin undergoes hepatic metabolism primarily via the cytochrome P450 system (CYP3A4), producing inactive metabolites such as clindamycin sulfoxide.
Elimination
The elimination half-life of clindamycin ranges from about 2.4 to 4 hours depending on age and health status. Approximately 10% of the drug is excreted unchanged in urine; most is eliminated as inactive metabolites in feces.
Special Considerations
Elderly patients may experience a slightly prolonged half-life but do not require dosage adjustments if they have normal liver function. Caution is advised when administering clindamycin alongside drugs that induce hepatic enzymes, as this may affect its clearance.
