Description	Apalutamide, also known as ARN-509 and JNJ-56021927 , is an androgen receptor antagonist with potential antineoplastic activity. ARN-509 binds to AR in target tissues thereby preventing androgen-induced receptor activation and facilitating the formation of inactive complexes that cannot be translocated to the nucleus. This prevents binding to and transcription of AR-responsive genes. This ultimately inhibits the expression of genes that regulate prostate cancer cell proliferation and may lead to an inhibition of cell growth in AR-expressing tumor cells.

Apalutamide is a nonsteroidal androgen receptor (AR) antagonist used primarily for the treatment of prostate cancer, particularly in patients with non-metastatic castration-resistant prostate cancer (nmCRPC). It is marketed under the brand name Erleada. Apalutamide works by blocking the action of androgens like testosterone and dihydrotestosterone on prostate cancer cells, thereby inhibiting tumor growth.

Mechanism of Action

Apalutamide selectively binds to the ligand-binding domain of the androgen receptor (AR), preventing its nuclear translocation and subsequent binding to DNA. This action disrupts the transcription of AR target genes involved in prostate cancer progression. Apalutamide has a higher affinity for the AR compared to other antiandrogens, such as bicalutamide, and retains its antagonist activity even in cases of increased AR expression.

Applications

Non-Metastatic Castration-Resistant Prostate Cancer (nmCRPC): It is used to delay the development of metastasis and improve overall survival.
Castration-Sensitive Prostate Cancer: In some cases, it may be used in combination with other therapies for advanced prostate cancer.
