    Fludarabine phosphate More Info

Fludarabine phosphate is a chemotherapeutic agent primarily used to treat hematological malignancies, specifically chronic lymphocytic leukemia (CLL) and certain types of non-Hodgkin lymphomas. As a purine analog, it interferes with DNA synthesis and repair by mimicking natural purine nucleotides. It is often used in both frontline and salvage therapies and has immunosuppressive properties, which also make it relevant in hematopoietic stem cell transplantation.

    Drug classification:

    Class: Antimetabolite chemotherapeutic (purine analog)

Common route of administration: Intravenous (IV); an oral form also exists

    Common brand name: Fludara®

    Mechanism of Action

Fludarabine phosphate is a prodrug. Once inside the body, it is rapidly dephosphorylated in the plasma to fludarabine (2-fluoro-ara-A). This compound is then transported into cells and rephosphorylated to the active triphosphate form, 2-fluoro-ara-ATP.

The active triphosphate inhibits several vital cellular functions:

Inhibits DNA polymerase α: Blocks DNA chain elongation

Inhibits ribonucleotide reductase: Depletes deoxynucleotide pools required for DNA synthesis

Inhibits DNA primase and ligase: Prevents DNA replication and repair

Incorporation into DNA and RNA: Causes chain termination and faulty RNA processing

The end result is impaired DNA synthesis and apoptosis (programmed cell death), particularly in rapidly dividing cells such as certain lymphocytes.

    Indications and Uses

Fludarabine phosphate is primarily indicated for:

Chronic lymphocytic leukemia (CLL), especially in patients who have not responded to alkylating agent-type chemotherapy

    Low-grade or indolent non-Hodgkin lymphomas

    Hairy cell leukemia (off-label)

Acute leukemias (in combination regimens, off-label)

Conditioning regimens prior to hematopoietic stem cell transplantation (HSCT), often in combination with busulfan or cyclophosphamide, due to its lymphodepleting effects

    Dosage and Administration

Fludarabine phosphate can be administered via intravenous infusion or as an oral tablet (availability may depend on region).

    Typical IV dosage (for CLL):

    25 mg/m²/day for 5 consecutive days every 28 days (1 cycle)

    Generally administered for 4–6 cycles

Oral dosage (if available): approximately 40 mg/m²/day for 5 days

Dosage must be adjusted in patients with renal impairment, and careful monitoring is essential to avoid cumulative toxicity.

    Pharmacokinetics

Rapidly dephosphorylated in plasma to fludarabine

High intracellular rephosphorylation to active form (2-F-ara-ATP)

Plasma half-life of fludarabine: approximately 10 hours

Primarily excreted by the kidneys – dose adjustment required in renal dysfunction

Crosses the blood-brain barrier (BBB): CNS toxicity is possible at high doses

    Side Effects and Toxicities

    Common adverse effects:

    Myelosuppression (dose-limiting toxicity)

    Neutropenia

    Thrombocytopenia

    Anemia

Increased risk of serious infections (due to immunosuppression)

Opportunistic infections like Pneumocystis jirovecii pneumonia (PJP), CMV, fungal infections

    Nausea, vomiting, fatigue

    Fever

    Serious or rare side effects:

Autoimmune hemolytic anemia (potentially severe or fatal)

Severe lymphopenia and T-cell suppression (increased risk of infection)

Tumor lysis syndrome (rapid breakdown of cancer cells)

    Neurologic toxicity (at high doses)

    Encephalopathy

    Seizures

    Pulmonary toxicity (e.g., interstitial pneumonitis)

Prophylactic antibiotics and antivirals may be given during therapy to prevent infections.

    Contraindications and Precautions

Contraindicated in patients with severe renal impairment (CrCl < 30 mL/min) without dose adjustment

Use caution in patients with preexisting myelosuppression or autoimmune conditions

Live vaccines are contraindicated during and shortly after treatment

Pregnancy category D – fludarabine is teratogenic; contraception required during and after treatment

    Drug Interactions

Pentostatin (another purine analog): Concomitant use may result in fatal pulmonary toxicity – contraindicated

Other immunosuppressive agents or recent administration of live vaccines increase risk of serious infection

Agents impairing renal excretion (e.g., probenecid): may affect drug levels

Special Considerations and Use in Transplantation

Fludarabine has become a key component in reduced-intensity conditioning (RIC) regimens for allogeneic stem cell transplantation

Offers profound T-cell immunosuppression, allowing donor cell engraftment with reduced toxicity

Often used in combination with busulfan, cyclophosphamide, ATG, or total body irradiation (TBI)

    Research and Emerging Uses

Investigated in combination therapies for high-risk or refractory leukemias and lymphomas

Used in chronic graft-versus-host disease (GVHD) protocols due to its immunosuppressive effects

Part of experimental conditioning regimens for gene therapy in immune disorders (e.g., SCID)

Research continues to optimize dosing and combinations to reduce toxicity

    Conclusion

Fludarabine phosphate is an important antimetabolite with proven efficacy in hematologic malignancies, particularly chronic lymphocytic leukemia. Its mechanism of action disrupts DNA synthesis and repair, leading to cell death, especially in lymphocytes. While effective, it carries significant risk of immunosuppression and bone marrow suppression, requiring careful monitoring. The drug’s role extends beyond cancer treatment, playing a vital part in transplant medicine due to its potent immunomodulatory properties.
