Fusidic acid is a steroid antibiotic primarily used to treat infections caused by susceptible Gram-positive bacteria, notably Staphylococcus aureus, including some methicillin-resistant strains (MRSA).

Mechanism of Action

Fusidic acid inhibits bacterial protein synthesis by targeting elongation factor G (EF-G) on the ribosome. It binds to EF-G after GTP hydrolysis during the translocation step of protein elongation, preventing the necessary conformational changes required for EF-G release from the ribosome. This effectively blocks bacterial protein synthesis by halting translocation and ribosome recycling, leading to inhibition of bacterial growth.

Applications

Skin and Soft Tissue Infections: Treatment of bacterial skin infections such as impetigo, cellulitis, and infected wounds.
Eye Infections: Used in conjunctivitis and other superficial eye infections.
MRSA Infections: Effective against many strains of methicillin-resistant Staphylococcus aureus, although monotherapy is discouraged due to rapid resistance development; combination therapy (e.g., with rifampicin) is preferred for serious infections.
Other Uses: Occasionally used topically for acne vulgaris, though less effective against Cutibacterium acnes compared to other antibiotics.

Elimination: Mainly excreted via bile into feces; renal excretion is minimal.
Half-life: Ranges from 3 to 6 hours depending on dose and administration route, necessitating multiple daily doses to maintain therapeutic levels.

Safety and Side Effects

Common Side Effects: Mild gastrointestinal discomfort, diarrhea, rash, and itching.
Serious Side Effects: Rare hepatotoxicity (liver function abnormalities), allergic reactions, and very rarely severe skin reactions such as Lyell’s syndrome.
Resistance: Resistance can develop rapidly with monotherapy, especially topical use, due to mutations in the EF-G target or resistance proteins like FusB that can dislodge fusidic acid from the ribosome.
Precautions: Monitoring liver function during prolonged therapy is advised. Avoid monotherapy for serious infections to reduce resistance risk.
Pregnancy and Lactation: Safety data are limited; use only if clearly needed.
