Description	Pristinamycin, also known as Virginiamycin antibiotic complex; NSC-246121; Antibiotic 899; Founderguard, is an antibiotic used primarily in the treatment of staphylococcal infections, and to a lesser extent streptococcal infections. Pristinamycin is a mixture of two components that have a synergistic antibacterial action. Pristinamycin IA is a macrolide, and results in pristinamycin's having a similar spectrum of action to erythromycin. Virginiamycin complex contains two streptogramin antibiotics, virginiamycin M1 (75%) and virginiamycin S1 (25%).

Pristinamycin More Info

Pristinamycin is a naturally occurring streptogramin antibiotic derived from the bacterium Streptomyces pristinaespiralis. It is primarily used to treat staphylococcal infections, including those caused by methicillin-resistant Staphylococcus aureus (MRSA), and to a lesser extent, streptococcal infections.

Composition and Mechanism

Pristinamycin is a combination of two structurally distinct components:

Pristinamycin IA (streptogramin B): a depsipeptide.

Pristinamycin IIA (streptogramin A): a polyunsaturated macrolactone.

Each component binds to the 50S ribosomal subunit of bacteria, inhibiting protein synthesis. When used individually, they exhibit bacteriostatic activity. However, in combination, they act synergistically, producing bactericidal effects up to 100 times more potent than either component alone.

Clinical Use

Primary Indications: Effective against erythromycin-resistant staphylococci and streptococci, and MRSA infections.

Formulation: Available orally as tablets (Pyostacine, marketed by Sanofi-Aventis in Europe), but not intravenously due to poor solubility.

Dosage: Typically 2–4 g per day orally, divided into three doses, taken with meals.

Special Applications: Used for bone and joint infections, particularly prosthetic joint infections, when other oral options are limited or unsuitable.

Adverse Effects

Common: Gastrointestinal disturbances and rash.

Rare but Serious: Toxic epidermal necrolysis, acute generalized exanthematous pustulosis, and allergic contact dermatitis.

Limitations

Geographic Availability: Licensed and widely used in Europe for over 30 years, but not licensed in the UK and considered restricted.

Administration: Oral only, which may limit use in severe infections requiring intravenous therapy.

Derivatives

Due to its poor solubility, a water-soluble derivative, quinupristin/dalfopristin (Synercid), was developed for intravenous use in serious infections
