Carbetocin is a long-acting, heat-stable analogue of oxytocin used to prevent postpartum hemorrhage by causing the uterus to contract after childbirth. It is administered intravenously or intramuscularly and its safety profile is comparable to oxytocin, with side effects including nausea, headache, and flushing. While its use in cesarean sections for PPH prevention has been evaluated, its non-inferiority compared to oxytocin for vaginal delivery varies by study and primary outcome.

Application

The primary use is to prevent uterine atony and postpartum hemorrhage (excessive bleeding after childbirth).

It is specifically indicated for prevention after a cesarean section under epidural or spinal anesthesia.

How it works

Carbetocin binds to oxytocin receptors on the smooth muscle of the uterus.

This binding causes rhythmic contractions of the uterus, increases uterine tone, and prolongs the duration and amplitude of contractions after delivery.

Side Effects

Common side effects can include nausea, abdominal pain, headache, back pain, and flushing or feeling of warmth.
Less common side effects may include a metallic taste and sweating.

In rare cases, more serious side effects like chest pain, fast heartbeat, or shortness of breath can occur.
