The antibacterial mechanism of ampicillin is identical to that of other beta-lactam antibiotics. It acts as a bactericidal agent by irreversibly inhibiting specific penicillin-binding proteins located in the bacterial cell membrane. These PBPs are enzymes critical for the final cross-linking step of peptidoglycan synthesis during cell wall formation. Inhibition disrupts cell wall integrity, leading to osmotic instability and eventual cell lysis, particularly in actively dividing bacteria. Its efficacy is nullified by bacterial production of beta-lactamase enzymes.

Ampicillin exhibits a broad spectrum of activity against a range of Gram-positive and Gram-negative bacteria. It is effective against many streptococci, non-beta-lactamase-producing staphylococci, and Listeria monocytogenes. Its defining advantage over penicillin G is its activity against certain Gram-negative bacilli, including many strains of Escherichia coli, Proteus mirabilis, Salmonella, Shigella, and Haemophilus influenzae (non-beta-lactamase producing). However, it remains inactive against Pseudomonas aeruginosa and most Klebsiella species.

Clinically, ampicillin is utilized for treating various infections caused by susceptible organisms. Common indications include urinary tract infections, respiratory tract infections such as bronchitis and pneumonia, bacterial meningitis (often in combination with other antibiotics), gastrointestinal infections caused by Salmonella or Shigella, and otitis media. It is a cornerstone agent for the treatment and prevention of infections caused by Listeria. It is available in formulations for oral administration, as well as for intravenous and intramuscular injection for systemic therapy.

The safety profile of ampicillin shares the common features of penicillins. The most frequent adverse effects are gastrointestinal disturbances, including diarrhea, which can be severe due to its impact on gut flora, and nausea.
