Captopril is the first orally active angiotensin-converting enzyme (ACE) inhibitor, a class of medications widely used in the treatment of cardiovascular diseases. It was developed in the 1970s and approved for medical use in the United States in 1981, revolutionizing the management of hypertension and heart failure.

Mechanism of Action

Captopril exerts its therapeutic effects by competitively inhibiting angiotensin-converting enzyme, a key enzyme in the renin-angiotensin-aldosterone system (RAAS). By blocking ACE, it prevents the conversion of angiotensin I to angiotensin II—a potent vasoconstrictor that also stimulates aldosterone secretion. Reduced angiotensin II levels lead to systemic vasodilation, lowering peripheral vascular resistance and blood pressure. Additionally, decreased aldosterone release reduces sodium and water retention, further contributing to blood pressure control and alleviating cardiac workload.

Indications

Hypertension

It is used as a first-line or adjunctive treatment for essential hypertension (high blood pressure of unknown cause), effectively lowering blood pressure in both mild-to-moderate and severe cases.

Heart Failure

Administered in combination with diuretics and beta-blockers to improve symptoms, enhance exercise tolerance, and reduce mortality in patients with congestive heart failure.

Myocardial Infarction

Used in the early post-infarction period (within 24 hours) for patients with left ventricular dysfunction to prevent ventricular remodeling and improve long-term prognosis.

Diabetic Nephropathy

Recommended for patients with type 1 diabetes and microalbuminuria, as it slows the progression of renal damage by reducing intraglomerular pressure.

Dosage and Administration

Captopril is available in oral tablets, typically in strengths of 12.5 mg, 25 mg, 50 mg, and 100 mg. The dosage is individualized based on the indication and patient response:

For hypertension: The initial dose is usually 12.5 mg twice daily, titrated up to a maximum of 150 mg per day, divided into two or three doses.

For heart failure: The starting dose is 6.25 mg three times daily, gradually increased to a target dose of 50 mg three times daily as tolerated.

It is recommended to take captopril 1 hour before meals, as food can reduce its bioavailability by approximately 30–40%.

Adverse Effects

Common Adverse Effects

Hypotension: Especially common in patients with heart failure or those taking diuretics, which may cause dizziness, lightheadedness, or syncope (fainting), particularly after the first dose.

Dry Cough: A characteristic side effect of ACE inhibitors, occurring in 5–20% of patients, due to increased bradykinin levels in the respiratory tract. The cough is usually non-productive and resolves after discontinuing the medication.

Hyperkalemia: Increased serum potassium levels, more likely in patients with renal impairment or those taking potassium-sparing diuretics or potassium supplements.

Rare Adverse Effects

Angioedema: A severe, potentially life-threatening reaction characterized by swelling of the face, lips, tongue, or larynx, requiring immediate medical attention.

Renal dysfunction: Worsening of renal function, especially in patients with bilateral renal artery stenosis.

Skin rashes: Maculopapular rashes, sometimes accompanied by pruritus (itching); rarely, Stevens-Johnson syndrome or toxic epidermal necrolysis may occur.

Contraindications

Hypersensitivity to captopril or any other ACE inhibitor.

History of angioedema associated with ACE inhibitor use.

Pregnancy (especially second and third trimesters), as it may cause fetal harm, including renal failure, hypotension, and fetal growth restriction.

Bilateral renal artery stenosis or unilateral renal artery stenosis in a solitary kidney.

Precautions

Monitor blood pressure, serum potassium, and renal function regularly during treatment, especially after dose adjustments.

Use cautiously in patients with renal impairment, hepatic dysfunction, or volume depletion (e.g., due to diuretic therapy or diarrhea/vomiting).

Avoid abrupt withdrawal, as it may lead to rebound hypertension.

Drug Interactions

Diuretics: Potentiate the hypotensive effect of captopril; diuretic dose may need to be reduced or discontinued 2–3 days before initiating captopril to minimize the risk of first-dose hypotension.

Potassium-sparing diuretics (e.g., spironolactone) and potassium supplements: Increase the risk of hyperkalemia; concurrent use should be avoided or closely monitored.

Nonsteroidal anti-inflammatory drugs (NSAIDs): May reduce the antihypertensive effect of captopril and increase the risk of renal dysfunction, especially in elderly patients or those with renal impairment.

Lithium: Captopril may increase lithium levels, potentially causing lithium toxicity; serum lithium concentrations should be monitored closely.
