    Mechanism of Action

    Fluconazole exerts its antifungal effects by selectively inhibiting the enzyme lanosterol 14-α-demethylase, a cytochrome P450 enzyme crucial for converting lanosterol to ergosterol, an essential component of the fungal cell membrane. This inhibition disrupts ergosterol synthesis, leading to increased membrane permeability and ultimately fungal cell death. The accumulation of toxic sterols in the fungal membrane contributes to its fungistatic activity, although at higher concentrations, fluconazole can exhibit fungicidal properties against certain fungi, particularly Cryptococcus species.

    Applications

    Fluconazole is indicated for the treatment of various fungal infections, including:
    Candidiasis: Oropharyngeal, esophageal, and systemic candidiasis.
    Cryptococcal Meningitis: Particularly in immunocompromised patients such as those with HIV/AIDS.

    Metabolism: Minimal hepatic metabolism occurs; only about 10% is metabolized. The drug is primarily eliminated unchanged via renal excretion (approximately 80%).
    Half-Life: The elimination half-life is approximately 30 hours, allowing for once-daily dosing in most cases. Steady-state concentrations are typically reached within 7 days of regular dosing.
