    Lopinavir More Info

Lopinavir is a protease inhibitor primarily used in the treatment of human immunodeficiency virus (HIV) infection. It works by blocking the HIV protease enzyme, which is essential for the virus to replicate and mature in host cells. By inhibiting this enzyme, lopinavir prevents the production of infectious viral particles, helping to reduce the viral load in the body and slow the progression of HIV disease.

    Key Characteristics

Formulation and AdministrationLopinavir is almost always used in combination with ritonavir (a low-dose booster protease inhibitor). Ritonavir inhibits the metabolism of lopinavir, thereby increasing its blood concentration and prolonging its therapeutic effect. The combined formulation is available as oral tablets or oral solution. It is usually taken twice daily with food to enhance absorption.

Mechanism of ActionHIV protease is responsible for cleaving viral polyproteins into functional proteins required for viral assembly. Lopinavir binds to the active site of the protease enzyme, preventing this cleavage process. As a result, the virus produces non-infectious, immature viral particles that cannot infect new cells.

    Indications

Treatment of HIV-1 infection in adults and pediatric patients (children over a certain age and weight range).

Often part of highly active antiretroviral therapy (HAART) regimens, combined with other antiretroviral drugs to maximize efficacy and reduce the risk of drug resistance.

Adverse EffectsCommon side effects include gastrointestinal symptoms such as nausea, vomiting, diarrhea, and abdominal pain. These symptoms are usually mild to moderate and may improve over time. Long-term use may be associated with metabolic abnormalities, including elevated cholesterol and triglyceride levels, insulin resistance, and lipodystrophy (abnormal fat redistribution in the body). Rare but serious side effects include liver toxicity and cardiac conduction abnormalities (e.g., prolonged QT interval).

Drug InteractionsLopinavir/ritonavir is metabolized by the cytochrome P450 3A4 (CYP3A4) enzyme system. It can interact with many other drugs:

Contraindicated drugs: Certain antiarrhythmics, sedatives, and ergot alkaloids, which may lead to serious or life-threatening reactions.

Drugs requiring dose adjustment: Antifungals, antibiotics, and other antiretroviral agents, as co-administration may alter their blood levels.

Resistance ConsiderationsThe development of HIV drug resistance is a major concern. To minimize this risk, lopinavir must be used in combination with other effective antiretroviral drugs, and patients should adhere strictly to their treatment regimen (avoid missing doses).

    Clinical Significance

Since its approval, lopinavir/ritonavir has been a cornerstone of HIV treatment, especially in resource-limited settings due to its efficacy and relatively manageable safety profile. It has also been studied for potential use in treating other viral infections, such as severe acute respiratory syndrome (SARS), though its primary indication remains HIV therapy.
