Ritonavir is an antiviral medication used in combination with other drugs to treat HIV-1 and COVID-19. It is a protease inhibitor, but its main use is to "boost" the levels of other antiviral drugs.

Mechanism of action

As an antiviral: Ritonavir, sold under the brand name Norvir, is a protease inhibitor that binds to the active site of the HIV protease enzyme. By blocking this enzyme, it prevents HIV polyproteins from being cleaved into functional parts needed to create new viral particles. This was its original purpose for treating HIV.

As a booster: When taken in low doses, ritonavir acts as a potent inhibitor of the human liver enzyme cytochrome P450 3A4 (CYP3A4). This enzyme is responsible for metabolizing many drugs. By inhibiting CYP3A4, ritonavir prevents the breakdown of other antiviral medications, increasing their concentration in the body and boosting their effectiveness.
