    1. Description

    Rapamycin (RAPA) is a novel macrolide immunosuppressant. It is a white crystalline solid with a melting point of 183–185°C. It is lipophilic and soluble in organic solvents such as methanol, ethanol, acetone, and chloroform; it is very slightly soluble in water and practically insoluble in diethyl ether. Developed as early as the 1970s, it was initially used as a low-toxicity antifungal agent. In 1977, its immunosuppressive properties were discovered, and in 1989, RAPA began to be tested as a new drug for treating rejection reactions in organ transplants. Based on the results of animal experiments and clinical applications, it is a novel immunosuppressant with good efficacy, low toxicity, and no nephrotoxicity. It is now commonly used as a drug to maintain immune tolerance in transplant recipients (particularly in kidney transplants) to mitigate immune rejection following organ transplantation. However, scientists have recently discovered another potential use for it: the treatment of Alzheimer’s disease (senile dementia). What has particularly piqued their interest is that the main component of rapamycin is also found in bacterial metabolites isolated from the soil of Easter Island. The latest experiments indicate that administering this substance to experimental mice with the condition can restore impaired memory function.

    2. Application

    (1) Prevention of organ transplant rejection: Used as an immunosuppressant to prevent rejection reactions following organ transplants, such as kidney transplants, it is typically administered in combination with cyclosporine and corticosteroids. The FDA approved its use for this indication in 1999. Its mechanism of action involves inhibiting the activation and proliferation of T lymphocytes in response to antigen and cytokine stimulation.

    (2) Anticancer Therapy: Rapamycin and its derivatives (such as everolimus and temsirolimus) are being studied for the treatment of various cancers and have been approved by the FDA as anticancer drugs. Research indicates that they are effective against diseases such as lymphangioleiomyomatosis.

    (3) Anti-aging and longevity research: As one of the most closely watched anti-aging drugs today, rapamycin has been shown to extend lifespan and improve health in various animal models, including yeast, nematodes, fruit flies, and mice. A landmark study in 2009 found that rapamycin significantly extended lifespan even when administered to mice in their later stages of life. Studies indicate that low-dose Rapamycin can improve immune responses, markers of skin aging, cardiac function, and cognitive ability in elderly individuals.

    (4) Dermatological Research: Topical Rapamycin (e.g., HYFTOR®) has been approved for the treatment of facial angiofibromas associated with tuberous sclerosis. Furthermore, off-label topical studies have demonstrated its potential in treating various skin conditions, including hereditary skin disorders (such as congenital onychodysplasia and epidermolysis bullosa simplex), benign proliferative diseases (such as trichilemmal tumors), and Kaposi’s sarcoma.

    (5) Autoimmune Disease Research: It has demonstrated immunosuppressive effects in animal models of systemic lupus erythematosus, collagen-induced arthritis, autoimmune diabetes, and graft-versus-host disease.

    3. Main Efficacy

    (1) Inhibition of mTOR kinase activity: Rapamycin forms a complex with FKBP12 that specifically inhibits the activity of mTORC1. mTORC1 is a central kinase that regulates protein synthesis, cell growth, and metabolism; its substrates include translational regulators such as S6K1 and 4E-BP1. By inhibiting mTORC1, rapamycin blocks the progression of the cell cycle from the G1 phase to the S phase. This mechanism of action underlies its immunosuppressive and antitumor effects.

    (2) Induction of autophagy: mTORC1 acts as a negative regulator of autophagy. By inhibiting mTORC1, rapamycin releases the inhibition of the ULK1 complex, thereby activating the autophagy process and promoting the clearance of damaged proteins and organelles. The restoration of autophagy activity is considered one of the key mechanisms by which rapamycin extends lifespan and improves age-related functional decline.

    (3) Delaying Aging and Extending Healthy Lifespan: By inhibiting the mTORC1 signaling pathway, rapamycin shifts cellular metabolism from a synthetic state to a maintenance and repair state. Studies indicate that its anti-aging mechanisms involve multiple aspects: improving protein homeostasis (reducing translation errors and toxic protein aggregation), promoting mitochondrial biogenesis and energy metabolism, suppressing chronic inflammation (reducing NF-κB and NLRP3 inflammasome activity), and maintaining stem cell function and tissue regenerative capacity. In immune cells of the elderly, low-dose rapamycin exerts an anti-aging effect by directly protecting genomic DNA and reducing the expression of p21, a marker of DNA damage-induced aging.

    (4) Inhibition of cell proliferation and tumor growth: By blocking downstream signaling pathways of mTORC1 (such as S6K1 and 4E-BP1), it inhibits protein synthesis and proliferation in tumor cells. In various tumor models, rapamycin and its derivatives have demonstrated tumor growth-inhibiting effects.

    (5) Immunomodulatory Effects: Rapamycin’s effects on the immune system are dose-dependent. At high doses, it exerts immunosuppressive effects (inhibiting T-cell proliferation and reducing pro-inflammatory cytokines); at low doses, it may have immunostimulatory effects (increasing the number of CD8+ memory T cells and enhancing vaccine responses). This characteristic gives it unique advantages in anti-aging and improving immune function.
