Bretazenil, also known as Ro 16-6028, is a benzodiazepine receptor partial agonist belonging to the imidazobenzodiazepine class of compounds-2-8. It was originally developed by Roche as a potential treatment for anxiety disorders and was evaluated in clinical trials up to Phase III-1-10. Unlike traditional benzodiazepines such as diazepam, bretazenil acts as a partial agonist at the GABA-A receptor complex, which gives it a distinct pharmacological profile-3-8. Preclinical studies demonstrated that bretazenil exhibits potent anxiolytic and vulsant properties with significantly reduced sedative, muscle relaxant, and abuse liability compared to full agonists-3-8. The compound binds to a broader range of GABA-A receptor subtypes than classical benzodiazepines, including those containing α4 and α6 subunits, contributing to its unique efficacy profile-7.
