    Monosodium glutamate (MSG) is the sodium salt of the non-essential amino acid L-glutamic acid, with the chemical formula C₅H₈NNaO₄ and a molecular weight of 169.11 g/mol. The compound was first extracted in 1908 by Japanese chemist Kikunae Ikeda from kombu seaweed broth and subsequently commercialized as a flavor enhancer.

    MSG is produced today through the natural fermentation of starch, sugar beets, sugar cane, or molasses. It is widely recognized by trade names including Aji-no-moto, Accent, and Vetsin.

    MSG is primarily used as a flavor enhancer, intensifying and enriching the savory "umami" taste in foods. It is commonly added to soups, sauces, broths, canned vegetables, deli meats, processed meats, snack foods, instant noodles, and seasoning blends. The compound enhances flavors when added to poultry, seafood, meats, and vegetables, though its effectiveness varies by food type. Typical usage levels range from 0.2% to 0.5% in food applications.

    Medical Applications

    MSG has been used therapeutically as an adjunctive treatment for hepatic encephalopathy and hepatic coma associated with elevated blood ammonia levels. In this context, glutamate binds with ammonia to form glutamine, helping to reduce blood ammonia concentrations. However, this use has declined due to limited efficacy and concerns about potential exacerbation of cerebral edema from glutamine accumulation. The typical pharmaceutical dosage is 11.5 g via intravenous infusion, not exceeding 23.0 g daily.

    Research Applications

    In laboratory settings, MSG (L-glutamic acid monosodium salt) is used as an excitatory amino acid neurotransmitter and an agonist of kainate, NMDA, and AMPA glutamate receptors in neuroscience research. It is also employed as a biochemical reagent and standard for analytical purposes.

    Safety and Regulatory Status

    Regulatory Approvals

    MSG is classified as Generally Recognized as Safe (GRAS) by the U.S. FDA and is approved as a food additive (E621) by the EU. JECFA has established an Acceptable Daily Intake (ADI) of 0–0.15 g/kg body weight.

    Adverse Reactions and "Chinese Restaurant Syndrome"

    MSG has been historically associated with a constellation of symptoms collectively termed "Chinese Restaurant Syndrome" (or Kwok's syndrome), originally described in 1968. Reported symptoms include:

    • Neurological and Muscular: Headache, back and neck pain, numbness, tingling, weakness

    • Systemic: Flushing, sweating, nausea, palpitations, chest tightness

    These symptoms typically onset 1–2 hours after ingestion of MSG-containing foods.

    However, controlled double-blind placebo-challenge studies have failed to consistently reproduce these symptoms when MSG is administered with food. The available evidence suggests that while a small subset of the population (less than 1%) may be particularly sensitive, responses are generally mild and transient, and require large doses (greater than 3 grams) consumed without food. Similar rates of symptom reporting are observed with placebo challenges.

    Asthma

    MSG has been investigated as a potential trigger for asthma exacerbations. The most robust studies, including a single-blind placebo-controlled challenge in 100 asthmatic patients, demonstrated no significant decline in lung function following MSG exposure, even in individuals with a history of reactions after Asian cuisine. A Cochrane review concluded insufficient evidence exists to recommend MSG avoidance in adults with asthma.

    Sodium Content Consideration

    MSG contains approximately 12.28 g of sodium per 100 g, about one-third the sodium content of table salt (39.34 g/100 g). Replacing half a teaspoon of table salt with MSG can reduce dietary sodium by approximately 37% while maintaining perceived saltiness.
