Methylene blue trihydrate (C.I. Basic Blue 9 trihydrate) is a guanylyl cyclase (sGC), monoamine oxidase A (MAO-A) and NO synthase (NOS) inhibitor. Methylene blue trihydrate is a vasopressor and is often used as a dye in several medical procedures. Methylene blue stains to negatively charged cell components like nucleic acids; when administered in the lymphatic bed of a tumor during oncologic surgery, methylene blue may stain lymph nodes draining from the tumor, thereby aiding in the visual localization of tumor sentinel lymph nodes. When administered intravenously in low doses, this agent may convert methemoglobin to hemoglobin.|A compound consisting of dark green crystals or crystalline powder, having a bronze-like luster. Solutions in water or alcohol have a deep blue color. Methylene blue is used as a bacteriologic stain and as an indicator. It inhibits GUANYLATE CYCLASE, and has been used to treat cyanide poisoning and to lower levels of METHEMOGLOBIN.

METHYLENE BLUE TRIHYDRATE is incompatible with strong oxidizing agents, caustics, alkali metal iodides and reducing agents. Also incompatible with dichromates. It forms double salts with many inorganic salts. Is bleached reversibly by (zinc + hydrochloric acid) or sodium hydrosulfite. In aqueous solution, this chemical is decolorized by zinc dust and dilute sulfuric acid, but the color is restored upon exposure to air and more rapidly upon addition of ammonium hydroxide. SMALL SPILLS AND LEAKAGE: If you spill this chemical, you should dampen the solid spill material with water, then transfer the dampened material to a suitable container. Use absorbent paper dampened with water to pick up any remaining material. Seal your contaminated clothing and the absorbent paper in a vapor-tight plastic bag for eventual disposal. Wash all contaminated surfaces with a soap and water solution. Do not reenter the contaminated area until the Safety Officer (or other responsible person) has verified that the area has been properly cleaned. STORAGE PRECAUTIONS: You should store this chemical under ambient temperatures, and keep it away from oxidizing materials. (NTP, 1992)
RECOMMENDED RESPIRATOR: Where the neat test chemical is stored, weighed and diluted, wear an approved half face respirator equipped with an organic vapor/acid gas cartridge (specific for organic vapors, HCl, acid gas and SO2) with a dust/mist filter. Splash proof safety goggles should be worn while handling this chemical. Alternatively, a full face respirator, equipped as above, may be used to provide simultaneous eye and respiratory protection. Drug Information
Compounds or agents that combine with an enzyme in such a manner as to prevent the normal substrate-enzyme combination and the catalytic reaction. (See all compounds classified as Enzyme Inhibitors.)
SYMPTOMS: Symptoms of exposure to this compound via ingestion include nausea, vomiting, diarrhea and dysuria. Ingestion of large amounts may cause hemolytic anemia. This compound causes skin irritation and severe eye irritation. Eye contact may result in corneal and conjunctival injury, stinging sensation, epiphora and, rarely, transient spasm of accommodation and blue coloration of the vitreous humor. Intravenous exposure to large amounts may cause nausea, abdominal pain, chest pain, headache, dizziness, mental confusion and profuse sweating. Subcutaneous exposure may lead to necrotic abscesses. Intrathecal injection may cause quadriplegia. ACUTE/CHRONIC HAZARDS: This compound is toxic by ingestion. It causes severe eye irritation. When heated to decomposition it emits very toxic fumes of carbon monoxide, carbon dioxide, nitrogen oxides, sulfur oxides, chlorine and hydrogen chloride gas. (NTP, 1992)
EYES: First check the victim for contact lenses and remove if present. Flush victim's eyes with water or normal saline solution for 20 to 30 minutes while simultaneously calling a hospital or poison control center. Do not put any ointments, oils, or medication in the victim's eyes without specific instructions from a physician. IMMEDIATELY transport the victim after flushing eyes to a hospital even if no symptoms (such as redness or irritation) develop. SKIN: IMMEDIATELY flood affected skin with water while removing and isolating all contaminated clothing. Gently wash all affected skin areas thoroughly with soap and water. If symptoms such as redness or irritation develop, IMMEDIATELY call a physician and be prepared to transport the victim to a hospital for treatment. INHALATION: IMMEDIATELY leave the contaminated area; take deep breaths of fresh air. If symptoms (such as wheezing, coughing, shortness of breath, or burning in the mouth, throat, or chest) develop, call a physician and be prepared to transport the victim to a hospital. Provide proper respiratory protection to rescuers entering an unknown atmosphere. Whenever possible, Self-Contained Breathing Apparatus (SCBA) should be used; if not available, use a level of protection greater than or equal to that advised under Protective Clothing. INGESTION: If the victim is conscious and not convulsing, give 1 or 2 glasses of water to dilute the chemical and IMMEDIATELY call a hospital or poison control center. Be prepared to transport the victim to a hospital if advised by a physician. If the victim is convulsing or unconscious, do not give anything by mouth, ensure that the victim's airway is open and lay the victim on his/her side with the head lower than the body. DO NOT INDUCE VOMITING. IMMEDIATELY transport the victim to a hospital. (NTP, 1992)

It is mainly used for dyeing cotton, acrylic fiber, hemp, silk, paper, and also used for the coloring of bamboo and wood and for the production of inks and lakes. It can also be used for the dyeing of biological bacteria; used as redox indicator, adsorption indicator and biological Bacterial stains for staining purposes. Mix indicator for serum protein biochemical test. Prepare exfoliated cell test staining solution. Prepare alkaline methylene blue staining solution. Redox indicator. Determination of mercury and tin. Reducing titrant to precipitate perchlorate and rhenate. Work with others to measure selenium and molybdenum photometrically.

Drug Function and Efficacy
At low concentrations, methemoglobin can be reduced to normal hemoglobin; at high concentrations, normal hemoglobin can be oxidized to methemoglobin, thereby temporarily inhibiting the toxicity of CN- to tissues.
