On January 29th, Laekna Pharmaceuticals disclosed the topline data from the international multicenter Phase II clinical trial (PROfecta-II) evaluating the efficacy of afuresertib, in combination with paclitaxel, for platinum-resistant ovarian cancer (PROC) patients. However, despite the release of this data, Laekna Pharmaceuticals' stock price experienced a sharp decline of 35%. Afuresertib (LAE002) is an orally administered small molecule pan-AKT kinase inhibitor known for its potent inhibition of AKT1, AKT2, and AKT3 kinases, which can suppress the proliferation of tumor cell lines from various tissues, including breast, blood, colon, ovaries, and prostate.

 

Challenges in the AKT Inhibitor Industry:
The AKT inhibitor industry is still in its early stages, primarily focused on clinical research and development. Currently, only AstraZeneca's Truqap has received regulatory approval for market launch. Laekna Pharmaceuticals' afuresertib was previously ranked as the second most advanced AKT inhibitor in clinical development. This positioned Laekna Pharmaceuticals as a shining example of domestic AKT inhibitors. However, the recent disclosure of the PROfecta-II trial results highlighted some concerns. Although the combination therapy showed a reduction in the risk of disease progression or death (progression-free survival, PFS) among PROC patients, the results did not reach statistical significance, with a hazard ratio (HR) of 0.744 (95% CI: 0.502-1.102). Laekna Pharmaceuticals has stated that they plan to discuss the trial results with regulatory authorities to determine the next steps for the registration clinical pathway.

 

Implications for Laekna Pharmaceuticals:
The release of these results undoubtedly shook Laekna Pharmaceuticals and afuresertib, also known as LAE002, which is one of their flagship products. Laekna Pharmaceuticals, founded in 2016, focuses on the development of small molecule targeted therapies for cancer treatment. After its listing on the Hong Kong Stock Exchange in June 2023, the company currently possesses two core candidate products, LAE002 (an AKT potent inhibitor) and LAE001 (a testosterone synthesis inhibitor), along with 15 cancer pipeline products and three liver fibrosis pipeline products.

 

The Road Ahead:
While facing challenges, Laekna Pharmaceuticals has a robust product pipeline. LAE002 shows promise in treating various cancers, including ovarian, prostate, breast, and PD-1/PD-L1 resistant solid tumors. Among the five development pipelines derived from LAE002, the combination therapy with paclitaxel for PROC patients has shown the fastest progress. However, this setback raises crucial questions for Laekna Pharmaceuticals regarding their decision-making process moving forward.

 

The AKT Target and Global Market Landscape:
AKT represents a clinically promising target in the field of cancer treatment. The activation of the AKT pathway plays a significant role in cell survival, proliferation, migration, and differentiation, making it a potential target for combating various cancers such as ovarian, prostate, breast, gastric, multiple myeloma, and melanoma. Despite previous setbacks in AKT inhibitor development, the landscape has evolved, with nearly 30 AKT inhibitors currently in clinical development globally. In China, there are only eight AKT inhibitors at the clinical stage, indicating substantial opportunities in this market.

 

The sharp decline in Laekna Pharmaceuticals' stock price following the release of disappointing trial results reflects the challenges faced by the AKT inhibitor industry. Laekna Pharmaceuticals' afuresertib, once regarded as a beacon of domestic AKT inhibitors, now requires careful evaluation and decision-making regarding its future development and commercialization. As the industry continues to evolve, the global market for AKT inhibitors presents significant potential, and Laekna Pharmaceuticals must navigate this landscape to secure its position and regain investor confidence.
