| Sorbitol |
Sorbitol monosaccharide is an isomer of mannitol, and its effect is similar to that of mannitol but weaker. After intravenous injection of a concentrated solution (25%), except for a small part converted into sugar, most of it is excreted in its original form through the kidneys, forming blood hyperosmosis, which can dehydrate the surrounding tissues and brain parenchyma and be excreted from the urine along with the drug, thereby reducing intracranial pressure and eliminating edema. High efficacy appears 2 hours after injection, and the brain edema gradually subsides, the tension disappears, the cerebrospinal fluid pressure decreases, it is not metabolized in the body, and after glomerular filtration, it is rarely reabsorbed in the renal tubules, playing an osmotic diuretic role. (1) Tissue dehydration effect: Increases plasma colloidal osmotic pressure, causing water in tissues (including eyes, brain, cerebrospinal fluid, etc.) to enter the blood vessels, reducing tissue edema, reducing intraocular pressure, intracranial pressure, and cerebrospinal fluid volume and pressure. (2) Diuretic effect: ① Increase blood volume, promote the secretion of prostaglandin I2, dilate renal blood vessels, increase renal blood flow including renal medullary blood flow, dilate glomerular afferent arterioles, increase glomerular capillary pressure, and increase cortical glomerular filtration rate. ② After glomerular filtration, very little (10%) of this drug is reabsorbed by the renal tubules, increasing the osmotic concentration of the renal tubular fluid and reducing the reabsorption of water and Na+, Cl-, K+, Ca2+, Mg2+ and other solutes by the renal tubules. Since the renal tubular fluid flow increases after the infusion of sorbitol, when certain drugs and poisons are poisoned, the concentration of these substances in the renal tubules decreases, the toxicity to the kidneys is reduced, and the excretion through the kidneys is accelerated.
