Function and Efficacy

The antibacterial spectrum of this product is the same as that of lincomycin, and its antibacterial activity is 4 to 8 times stronger than that of lincomycin. This product has high antibacterial activity against aerobic Gram-positive cocci, such as Staphylococcus (including penicillin-resistant and methicillin-sensitive strains), hemolytic streptococci, grass-green streptococci, and Streptococcus pneumoniae. It also has good antibacterial effects on anaerobic bacteria. Most of the genera, such as Bacteroides, Fusobacterium, Actinomyces, Peptococcus, and Peptostreptococci, are sensitive to this product. This product acts on the 50S subunit of the ribosome of sensitive bacteria, preventing the extension of the peptide chain, thereby inhibiting the protein synthesis of bacterial cells. It is generally an antibacterial agent, but at high concentrations, it also has a bactericidal effect on certain bacteria.

Indication

This product is suitable for the following infections caused by sensitive strains of Streptococcus, Staphylococcus and anaerobic bacteria: otitis media, sinusitis, suppurative tonsillitis, pneumonia; skin and soft tissue infections; in the treatment of bone and joint infections, abdominal infections, pelvic infections, empyema, lung abscess, osteomyelitis, sepsis and other diseases, it can be used alone or in combination with other antibacterial drugs according to the situation.

Usage and Dosage

For adults, take 0.15-0.3g orally 4 times a day. For severe infection, the dosage can be increased to 0.45g orally 4 times a day. For children aged 4 weeks or older, take 8-16mg/kg per day, divided into 3-4 times.

Adverse Reactions

1 Gastrointestinal reactions: including nausea, vomiting, abdominal pain, diarrhea and other symptoms; severe cases include abdominal cramps, abdominal tenderness, severe diarrhea (watery or purulent), accompanied by fever, abnormal thirst and fatigue (pseudomembranous enterocolitis). Diarrhea, enteritis and pseudomembranous enterocolitis may occur during treatment or after drug withdrawal. 2 Allergic reactions: usually mild to moderate measles-like rash is the most common, followed by vesicular rash and urticaria, occasionally erythema multiforme, exfoliative dermatitis, and some manifestations of Stevens-Johnson syndrome. 3 Abnormal liver function, abnormal kidney function, occasionally neutropenia and eosinophilia may occur.

Precautions

It is contraindicated in patients with a history of hypersensitivity to clindamycin and lincomycin.

Special Population Medication

Precautions for children: It is not suitable for children under 1 month old. Precautions for pregnancy and lactation: Animal experiments show that this product has no effect on the fetus, but there is still a lack of experience in its use in pregnant humans, so it should still be used with caution in pregnant women. This product can be secreted into breast milk, and lactating women must suspend breastfeeding if they really need to use the drug. Precautions for the elderly: There is not enough data on the elderly in the clinical studies of this product, so it is not certain whether the elderly react to this drug differently from young people. For young and elderly volunteers with normal liver and kidney function, oral or intravenous administration of clindamycin did not show clinically significant differences in pharmacokinetics. It has been reported that diarrhea and antibiotic-associated colitis are more common and more severe in the elderly. Therefore, elderly patients with diarrhea should be closely monitored.

Drug Interactions

1. It can enhance the neuromuscular blockade of inhaled anesthetics, leading to skeletal muscle weakness and respiratory depression or paralysis (apnea). Caution should be taken when using it together during or after surgery. Treatment with anticholinesterase drugs or calcium salts is expected to be effective. 2. Used together with antiperistaltic antidiarrheal drugs and antidiarrheal drugs containing white clay: Clindamycin may cause pseudomembranous colitis with severe watery diarrhea during the course of treatment or even a few weeks after the course of treatment. Because it can delay the excretion of colonic endotoxins, thereby causing prolonged and aggravated diarrhea, antiperistaltic antidiarrheal drugs should not be used together. When antidiarrheal drugs containing white clay and clindamycin are taken orally at the same time, the absorption of the latter will be significantly reduced. Therefore, the two should not be taken at the same time and a certain time interval (at least 2 hours) must be maintained. 3. This product has a neuromuscular blocking effect. When used together with anti-myasthenic drugs, the effect of the latter on skeletal muscles will be weakened. In order to control the symptoms of myasthenia gravis, the dose of anti-myasthenic drugs should be adjusted during the combined treatment. 4. Chloramphenicol or erythromycin can replace this product at the target site, or inhibit the latter from binding to the bacterial ribosome 50S subunit. In vitro tests show that this product has an antagonistic effect with erythromycin, so this product should not be used in combination with chloramphenicol or erythromycin. 5. When used in combination with opioid analgesics, the respiratory depression of this product and the central respiratory depression of opioids may lead to prolonged respiratory depression or respiratory paralysis (apnea) due to the cumulative phenomenon, so the patient must be closely observed or monitored. 6. This product can enhance the effect of neuromuscular blocking drugs, and the two should be avoided in combination.
