Appearance

This product is pink crystalline powder. The aqueous solution is unstable when placed on ice and decomposes rapidly under light.

Indication

1. Used for emergency hypotension in hypertensive emergencies, such as hypertensive crisis, hypertensive encephalopathy, malignant hypertension, paroxysmal hypertension before and after pheochromocytoma surgery, etc. It can also be used for controlled hypotension during surgical anesthesia. 2. Used for acute heart failure, including acute pulmonary edema. It is also used for acute heart failure with acute myocardial infarction or valvular (mitral or aortic) insufficiency.

Usage and Dosage

Dilute with 5% glucose solution and protect from light. The method of administration is generally continuous intravenous drip, and the dosage is adjusted according to blood pressure monitoring. The general speed is 0.5-8 μg/minute per kilogram of body weight. For those who have received other antihypertensive drugs, the dose should be small, and the maximum dose can be 40 μg/minute per kilogram of body weight. For patients with heart failure, it can be started from 20-40 μg/minute per kilogram of body weight, intravenous drip for 8-12 hours a day, ensure that the patient rests at night, and monitor the blood thiocyanate concentration.

Adverse Reactions

Nausea, vomiting, mental restlessness, muscle cramps, headache, rash, sweating, fever, etc. When used continuously in large doses, patients with liver and kidney damage may suffer from increased plasma cyanide and thiocyanate concentrations and poisoning. This product may cause hypothyroidism, methemoglobinemia, phlebitis and metabolic acidosis.

Precautions

It is contraindicated for pregnant women and patients with compensated hypertension such as arteriovenous shunt or aortic stenosis.

Precautions during pregnancy and lactation: Contraindicated for use by pregnant women.

Drug Interactions

1. When used with other antihypertensive drugs, blood pressure can drop dramatically. 2. When used with dobutamine, cardiac output can be increased and pulmonary capillary pressure can be reduced. 3. When used with sympathomimetic amines, the antihypertensive effect of this product is weakened.
