Cefotaxime is a third-generation cephalosporin with a broad antibacterial spectrum. It has strong activity against Gram-negative bacteria such as Escherichia coli, Proteus mirabilis, Klebsiella and Salmonella, and other Enterobacteriaceae. It also has a good effect on Proteus vulgaris and Citrobacter. Enterobacter cloacae and Enterobacter aerogenes are relatively resistant to this product. This product has no antibacterial activity against Pseudomonas aeruginosa and Alcaligenes. Cefotaxime has a strong effect on influenza bacilli, target=_blank Neisseria gonorrhoeae (including β-lactamase-producing strains), target=_blank Neisseria meningitidis and Moraxella catarrhalis. This product has poor antibacterial activity against Staphylococcus aureus, strong activity against Gram-positive cocci such as hemolytic Streptococcus and target=_blank Streptococcus pneumoniae, and Enterococcus is resistant to this product.

The main ingredient of this product is: cefotaxime sodium. Its chemical name is: (6R,7R)-3-[(acetyloxy)methyl]-7-[2-amino-4-thiazolyl-(methoxyimino)acetylamino]-8-oxo-5-thia-1-azabicyclo[4.2.0]oct-2-ene-2-carboxylic acid sodium salt. Molecular formula: C16H16N5NaO7S2 Molecular weight: 477.45

This product is white, off-white or slightly yellowish white crystalline powder.

It is suitable for respiratory tract infections, urinary tract infections, gastrointestinal infections, meningitis, sepsis, soft tissue infections, ENT infections, reproductive tract infections, orthopedic infections, etc. caused by sensitive bacteria. Cefotaxime can be used as the first choice drug for meningitis.

Adults: 2-6g per day, divided into 2-3 times of intravenous injection or intravenous drip; patients with severe infection: 2-3g every 6-8 hours, and the maximum daily dose shall not exceed 12g. For the treatment of uncomplicated pneumococcal pneumonia or acute urinary tract infection, 1g every 12 hours. For newborns aged 7 days or less, 50mg/kg every 12 hours, and for those born more than 7 days old, 50mg/kg every 8 hours. The dose for treating meningitis patients can be increased to 75mg/kg every 6 hours, all administered intravenously. Patients with severe renal impairment should reduce the dose appropriately when using this product. When the serum creatinine value exceeds 424?mol/L (4.8mg) or the creatinine clearance rate is less than 20ml/min, the maintenance dose of this product should be halved; when the serum creatinine exceeds 751?mol/L (8.5mg), the maintenance dose is 1/4 of the normal amount. For patients who need hemodialysis, 0.5-2g per day. However, one dose should be added after dialysis.

The incidence of adverse reactions is low, about 3-5%. 1. Rash, urticaria, itching, drug fever, pain at the injection site, phlebitis, diarrhea, nausea, vomiting, loss of appetite, etc. 2. Mild increase in alkaline phosphatase or serum aminotransferase, temporary increase in blood urea nitrogen and creatinine, etc. 3. Leukopenia, eosinophilia or thrombocytopenia, etc. 4. Occasionally, headache, numbness, dyspnea and facial flushing may occur. 5. Mucosal candidiasis may occur in very few patients.

This product is contraindicated in patients who are allergic to cephalosporins or have a history of anaphylactic shock or immediate reaction to penicillin.

Precautions during pregnancy and lactation: Use with caution in lactating women.

1. It has a synergistic effect with gentamicin or tobramycin against Pseudomonas aeruginosa; it has a synergistic effect with amikacin against Escherichia coli, target=_blank Klebsiella pneumoniae and Pseudomonas aeruginosa. 2. When used in combination with aminoglycoside antibiotics, renal function should be followed up during medication. 3. When large doses of cefotaxime are used in combination with strong diuretics, attention should be paid to changes in renal function. 4. Cefotaxime can be diluted with target=_blank sodium chloride injection or glucose solution, but cannot be mixed with sodium bicarbonate solution. 5. When used in combination with azlocillin or mezlocillin, the total clearance rate of this product can be reduced. If the two are used in combination, the dose should be appropriately reduced.
