Function and Efficacy
This product is a calcium channel blocker, and its action is related to the inhibition of calcium ion influx during myocardial and vascular smooth muscle depolarization. This product can effectively dilate the epicardial and subendocardial coronary arteries, relieve spontaneous angina pectoris or angina pectoris caused by coronary artery spasm induced by ergonovine; by slowing down the heart rate and lowering blood pressure, it reduces myocardial oxygen demand, increases exercise tolerance and relieves exertional angina pectoris. This product can relax vascular smooth muscle, reduce peripheral vascular resistance, and lower blood pressure; the extent of its blood pressure reduction is related to the degree of hypertension, and only slightly reduces blood pressure in people with normal blood pressure. This product has a negative inotropic effect and can slow down the conduction of the sinoatrial node and atrioventricular node.

Indication

Usage and Dosage
Oral administration: the initial dose is 60~120 mg/time, twice a day (2~4 tablets at a time, twice a day), and the average dose range is 240~360 mg/day.
Adverse Reactions
Common adverse reactions: edema, headache, nausea, dizziness, rash, weakness. Rare adverse reactions (cardiovascular system: angina pectoris, arrhythmia, atrioventricular block, bradycardia, bundle branch block, congestive heart failure, abnormal electrocardiogram, hypotension, palpitations, syncope, tachycardia, ventricular premature beats. Precautions

Drug Interactions
1. β-blockers: Studies have shown that the combination of diltiazem hydrochloride and β-blockers is well tolerated, but there is insufficient data in patients with left ventricular dysfunction and conduction dysfunction. This product can increase the bioavailability of propranolol by nearly 50%, so the dose of propranolol needs to be adjusted when starting or stopping the combination of the two drugs. 2. Cimetidine: Due to the inhibition of cytochrome P450 oxidase, the first-pass metabolism of this product is affected, which can significantly increase the peak blood concentration and the area under the drug-time curve of this product.
