Nitroglycerin patch (Ⅱ)
Function and Efficacy
Nitroglycerin has the effect of relaxing vascular smooth muscle. Its relaxing effect on post-capillary capacitance vessels and major arteries, especially the coronary arteries that are still responsive, is stronger than its effect on resistance vessels. Systemic vasodilation increases venous blood volume, reduces ventricle volume, filling pressure, and diastolic wall tension (reduced preload). Dilation of the major arteries close to the heart can reduce systemic resistance (reduced afterload) and arterial resistance. The reduction in ventricular radius and decreased ventricular wall tension respectively reduce myocardial oxygen consumption. The reduction in cardiac filling pressure increases blood flow in the subintimal layer of the heart wall that is sensitive to ischemia, thereby increasing myocardial oxygen supply. At the molecular level, nitroglycerin may act through the same mechanism as endothelium-derived relaxing factor (EDRF), that is, by increasing the concentration of cyclic guanosine monophosphate (CGMP).

Indication
Long-term treatment of coronary heart disease, prevention of angina attacks, and treatment of chronic heart failure in combination with digitalis and/or diuretics.
Usage and Dosage
Initially use 1 tablet daily, applied to the chest skin. The dosage can be increased as needed.
Adverse Reactions
Headaches may occur at the beginning of treatment, but they usually disappear after a few days of use. Patching with nitroglycerin can also cause a drop in blood pressure, especially when used for the first time. Nausea, vomiting, dizziness, weakness, increased heart rate, and skin flushing may also occur at the beginning of treatment. Skin allergies to nitroglycerin are rare, and patients may occasionally feel slight itching or burning at the site of application, and slight redness of the skin will disappear with the removal of the patch.
Precautions
It is contraindicated in patients with acute circulatory failure, severe hypotension, and acute myocardial infarction.

Drug Interactions
The simultaneous use of antihypertensive drugs, calcium antagonists, vasodilators, tricyclic antidepressants or alcohol may enhance the antihypertensive effect of this drug. If nitroglycerin and dihydroergotamine are used at the same time, the former can increase the blood concentration of the latter and enhance its effect.
Storage
The storage temperature should not exceed 25℃ and the validity period is 3 years.
