Tocainide Hydrochloride Tablets

It is a Class Ib antiarrhythmic drug and a lidocaine homologue. It mainly acts on Purkinje fibers and ventricular muscles, inhibiting Na influx and promoting K outflow; reducing the slope of phase 4 depolarization, thereby reducing automaticity; significantly shortening the action potential duration, relatively prolonging the effective refractory period and relative refractory period; reducing myocardial excitability; slowing down conduction velocity; and increasing the ventricular fibrillation threshold. It does not affect the function of the sinoatrial node; it does not affect the ventricular depolarization and repolarization time. The LD50 of oral tocainide is 800 mg/kg in mice, 1000 mg/kg in rats, and 230 mg/kg in guinea pigs.

This product is a sugar-coated tablet, which appears white after removing the sugar coating.

Used for the treatment of severe ventricular arrhythmias, including ventricular premature beats and ventricular tachycardia.

Oral administration: 0.2-0.4g at a time, once every 8-12 hours. Or use 0.4g first, repeat after 3-4 hours, and then maintain 0.2-0.4g at a time, once every 8-12 hours. Or follow the doctor's advice.

Adverse reactions are mostly mild and short-lived, and generally do not affect treatment. Common adverse reactions include anorexia, nausea, vomiting, constipation, etc. in the gastrointestinal system; dizziness, headache, drowsiness, sweating, tinnitus, tremor, etc. in the nervous system. Rashes are occasionally seen. The above reactions can disappear on their own after stopping the drug.

It is contraindicated for patients who are allergic to tocainide or aminoacylase local anesthetics. It is contraindicated for patients with II to III degree atrioventricular block who do not have a pacemaker installed. It is not used to treat patients with fatal ventricular arrhythmias.

Tocainide and lidocaine have similar pharmacological effects, so the combination of these two drugs can increase the occurrence of side effects. Tocainide combined with cimetidine, digoxin and warfarin does not cause serious clinical effects, and Tocainide is effective in both digoxinized and non-digoxinized patients. However, the combination of tocainide and metoprolol has an effect on pulmonary wedge pressure and cardiac index.
